Position Overview for the Case Management Assistant:

  • Transition of Care Coordination: Collaborate with Case Managers and Social Workers to assess patient needs, develop, and implement a discharge plan, ensuring all post-acute care services are in place, including referrals, authorizations, and transportation services for a smooth transition.
  • Monitoring & Communication: Monitor the progress of the discharge plan, communicate updates to the patient, family, and care team, and escalate any barriers to meeting transition goals to the appropriate parties, such as Case Managers and Social Workers, to ensure timely and efficient patient discharge.
  • Administrative & Documentation Support: Maintain accurate and up-to-date documentation in the electronic medical record (EMR), including capturing discharge details, patient preferences, and communications. Provide clerical support, such as copying, faxing, and data entry, to assist the Case Management team.
  • Customer Service & Patient Advocacy: Serve as a liaison between the patient, family, and healthcare providers, delivering excellent customer service by addressing concerns, resolving issues, and ensuring that patients and their families are informed and comfortable with the discharge process.
Required Skills for the Case Management Assistant:
  • Education & Experience: High school diploma or equivalent required, with at least 1 year of recent relevant experience in a healthcare setting, preferably in case management, care coordination, or discharge planning.
  • Communication & Interpersonal Skills: Strong oral and written communication skills, with the ability to work effectively with patients, families, healthcare providers, and external vendors. Must demonstrate tact, respect, and professionalism in all interactions.
  • Organizational & Time Management Skills: Ability to work independently and manage multiple tasks in a fast-paced environment with shifting priorities. Strong time management and organizational skills are essential for managing the discharge process and meeting departmental goals.
  • Technical Proficiency: Intermediate proficiency in PC and word processing skills, along with experience using electronic medical records (EMR) systems such as Allscripts. Must be comfortable with data entry, maintaining documentation, and performing clerical duties.
