Landmark Hospital of SW Florida has an exciting opportunity for Part Time (~ 20-24 hours a week) Case Manager to join our dedicated and professional team dedicated to our mission of providing exceptional care to medically complex patients.
Case Manager Summary of Duties: Our Case Managers are responsible for the overall coordination of the patient's plan of care and the resources utilized for the patient's stay. Assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual's health needs throughout the continuum of care. The Case Manager provides discharge planning through referrals and multidisciplinary planning. They liaise between the patient/family and the healthcare team to ensure necessary care is provided promptly and effectively. Relying on experience and judgment, the Case Manager is most successful when building relationships across the healthcare environment. Case managers perform a variety of tasks related to the coordination of care.
Case Managers enjoy a certain degree of creativity and latitude when ensuring that the patients' unique needs are met in the most appropriate, timely, and cost-effective manner possible (and also within regulatory requirements).
The ideal candidate must be highly detail and goal-oriented, with superior analytic, critical-thinking and time-management skills. They must have a positive and empathetic personality with the ability to work effectively with others as part of a multidisciplinary team in a complex care environment. It is also vital that they have excellent communication and interpersonal skills and consistently display a professional attitude.
Case Manager Requirements:
  • An active RN/LPN license (preferred) or a bachelor's degree in Social Work (master's degree preferred)
  • 1 year of Case Management or discharge planning experience
  • Ability to develop and maintain positive working relationships
  • LTAC/Acute Care Hospital experience is strongly preferred
  • Strong computer skills preferred
