Pioneer Healthcare Services is seeking an experienced Home Health Physical Therapy Assistant PTA looking to travel to San Oxnard, CA. Home Health Physical Therapy Assistants who work for Pioneer Healthcare Services are dedicated individuals looking to pave a new way in travel healthcare.
Responsibilities of a Home Health Physical Therapist Assistant PTA: Provide physician-prescribed physical therapy under a plan of care established by the PT Improve or minimize residual physical disabilities of the patient Return the patient to optimum and productive levels (within patient capability) Participate with all other healthcare personnel in patient care planning Perform all skilled procedures as ordered by the physician and according to the plan of care established by the PT Consult with PT regarding change in treatment Participate in educating the patient and the patient's family Participate in QAPI and in-service programs as assigned Attend all patient care conferences as scheduled Prepare medical records, and progress notes and update care plans for each patient visit in a timely manner as per policy"
