Encourages, implements and facilitates independence, inclusion, individuality and productivity.Position Duties:Manages group settings’ activities and supervises assigned Habilitation Staff. Completes face to face supervision and annual performance evaluations for Community Skills staff. (Community Skills Specialists & Community Skills Assistants).Participates as a team member to develop and implement participant’s program plans.Coordinates and provides direction to team members in the assessment of program participant’s needs and development of appropriate individualized program plans that are in compliance with agency policy as well as State and Federal regulations. Assures integration of services with residential providers.Develops and implements a participant interest tool to assist in developing meaningful and functional activities reflecting participants personal interests and needs. Implements community based training opportunities and facilitates use of public transportation and public facilities as appropriate.Trains and supervises Community Skills Specialists and Community Skills Assistants in job performance i.e. goal/outcome implementation.Participates in scheduling on-going in-service training for habilitation staff by interdisciplinary team.Provides direction and training to interdisciplinary team members and participates in informal orientation programs for habilitation staff.Participates in interviews, hires, evaluates, supervises, disciplines and recognizes staff as appropriate.Participates in screening new participants. Participates in group activity routines when needed.Schedules and attends all annuals, semi-annual and special conferences for participants.Reviews all participant records for accuracy and completeness.Ensures that habilitation staff are entering daily recording of participants activities, goals in Precision Care software and reviews accordingly. Ensures documentation of services delivered contemporaneously, attendance, progress notes, etc.Acts as a liaison to outside professionals regarding participant and their plan.Performs all other duties as assigned.QUALIFICATIONS:Bachelor’s degree in education or related field required.QIDP Status.Minimum 2 years experience working in a program for people with developmental disabilities.Supervisory experience preferred.Working knowledge of Microsoft Office Applications required, and must become knowledgeable of PrecisionCare software and other agency software programs i.e E-Time.Must be able to work outside as appropriate for the activity.Must be able to assist ADL Care (feed, personal hygiene, toileting) care as needed.Valid Driver’s license preferred.REQUIREMENT:Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency’s corporate compliance program, including the code of business conduct, the NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.
