- Home Health Experience within the last year
- 1 year of PT experience in a home health setting
- Ability to physically lift and assist patients in hands-on care
**Job Description:**
- Provide case management and direct patient care within a team environment.
- Administer comprehensive physical therapy modalities in the home setting.
- Assist the physician in evaluating the level of function, develop the plan of care, prepare clinical and progress notes, and participate in in-service programs.
- Consult with the family and other agency personnel as needed.
- Promote effective working relationships and work as part of the team to meet goals and objectives.
- Promote the organization's philosophy, mission, and policies to ensure quality care delivery.
- Utilize resources, supplies, and time efficiently to contain costs and increase the value of services.
- Foster positive, supportive, and respectful communication with all external sources, patients, families, and staff.
- Develop and revise plans of treatment and discharge plans in conjunction with patients, families, physicians, care delivery teams, and third-party payors.
- Provide an environment promoting respect for patients, their privacy, confidentiality, and property.
- Initiate appropriate rehabilitative programs, including home therapy programs such as gait training, transfers, ADLs, home modification, and equipment use.
- Evaluate patients' progress and make necessary adjustments to the physical therapy program.
- Instruct paraprofessional staff in body mechanics, equipment use, bed mobility, transfers, gait patterns, home exercise programs, etc.
- Supervise the quality of patient care services provided by the Home Health Agency and ensure compliance with the care plan.
- Engage in patient conferences and contribute to comprehensive coordinated plans of treatment.
- Cooperatively assist in the orientation of new rehabilitation personnel.
- Maintain current knowledge of available services and ensure resolution of potential problem areas.
- Recognize and celebrate professional growth and accomplishments among team members.
- Engage in professional development through participation in education programs and professional organizations.
- Responsible for achieving performance indicators established by management.
This position requires excellent communication and interpersonal skills, the ability to work with diverse socio-cultural backgrounds, and the capability to accept and utilize supervision effectively.
