PRN position available.
Ability to travel to cases as assigned in the SWFL region is required.
Home Health by Cypress at Home is expanding and looking for individuals to come on board to help grow our Medical Social Work service line!
Our MSW’s work closely with our patients, caring for their physical and emotional needs, while building lasting relationships.
This role provides a range of professional Social Work services addressing needs including, but not limited to:
Ongoing assessment services
Counseling
Emotional support
Home care planning
Patient/family education
Resource identification
Information and referral advocacy
Other supportive services

WHAT YOU WILL BE DOING:
Providing clinical social evaluations and planning appropriate interventions based on findings.
Consulting and coordinating with a team of care professionals to ensure highest level of care and level of independent living is achieved.
Maintaining patient comfort and safety.
Acting as an advocate for both the patient and family to help navigate the patient through the community system.
WHAT WE ARE LOOKING FOR:
Master’s degree from a school of social work accredited by the Council on Social Work Education.
A Licensed Social Worker in the State of Florida.
Community-based experience preferred.
Possess an interest and compassion for older adults.
Able to work independently and manage time effectively.
