Association House of Chicago is hiring a Transition Team Coordinator to join its team in Chicago, IL. This is a supervisory position with a flexible first-shift schedule.
The Transition Team Coordinator supervises transition team staff and provides daily task assignment and oversight to ensure quality care, accurate documentation, and staff productivity. This role will also manage their own caseload and assist participants in the housing search process. Additionally, they help purchase transition items (furniture, food, etc.) using designated funds, while emphasizing the importance of building natural supports for both transition and ongoing recovery.

Job Duties:
• Build collaborative relationships with nursing facility staff for pre-discharge and discharge planning
• Ensure participants have access to basic resources and necessary medical, dental, mental health, substance abuse, and social services
• Help participants identify and build natural supports for transition, treatment, and recovery
• Support participants in self-advocacy, decision-making, treatment planning, and review
• Assist in the housing search process, outreach to landlords, and the Bridge Subsidy application process for Permanent Supportive Housing
• Help participants purchase transition items and set up their apartments
• Provide crisis management services, ensure access to stabilization services, and follow duty to warn principles
Job Requirements:
• Bachelor’s degree in psychology or related field
• 1+ years of mental health or substance abuse field for adults
• 1+ years supervisory experience
• Valid driver’s license and reliable vehicle
