Job Summary:
Coordinates input from the team and provider practices to provide continuity of care across the care continuum for a defined
population.
Qualifications:
Education - Required
  • Bachelor's Degree in Social Work, Sociology, Psychology or related field.
Experience - Required
  • 4 years Medical or clinical setting.
Additional Qualifications
  • Training in clinical setting (hospital and/or ambulatory) performing social services
Essential Functions:
  • Assists practices to identify community resources in a defined geographic area.
  • Assesses social determinants of health needs and coordinates with other social services/agencies to ensure patient needs are met.
  • Complete financial assistance applications with patients (ex. Medicaid, Sliding Fee Scale, and other benefit programs)
  • Documents all interventions succinctly and in a timely fashion in the medical record.
  • Documents patient/family understanding of barriers to appropriate care and transition planning.
  • Provides input and participates in the development of the patient-centered multidisciplinary care management plan.
  • Evaluates ongoing effectiveness in meeting established care goals and revises plan, as needed.
  • Provides telephonic or face-to-face outreach to patients based on referrals from the team and provider practices.
