This includes performing outreach to members, answering inbound calls, and scheduling services. The coordinator serves as the primary point of contact for members, providers, and staff, resolving issues and documenting member records in accordance with current state and regulatory guidelines.Key Responsibilities:Member Outreach: Conduct outreach to members via phone to discuss care plan next steps, community resources, and health plan inquiries, ensuring ongoing education for both members and providers.Resource Connection: Assist members in connecting with health plan and community resources, facilitating high-quality customer care and service.Front-Line Support: Address various inquiries and requests from members and providers, explaining care plan procedures and protocols.Onboarding Support: Manage member onboarding and daily administrative tasks, including sending welcome letters, correspondence, and educational materials to enhance member/provider relationships.Record Documentation: Maintain non-clinical member records, ensuring compliance with state and regulatory standards, and provide documentation to providers as necessary.Social Determinants of Health (SDOH): Utilize knowledge of local benefits and resources to make referrals that address SDOH needs.Additional Duties: Perform other tasks as assigned and adhere to all policies and standards.Education/Experience:High School diploma or GED required.1-2 years of related experience preferred, ideally in a call center or healthcare setting.Required Skills:Proficiency in Microsoft Office Suite (Word, Excel, Outlook).Strong spoken communication skills.Typing proficiency.Prior call center experience.Experience in a managed health plan or medical facility.BenefitsBCforward offers all eligible employees a comprehensive benefits package including, but not limited to major medical, HSA, dental, vision, employer-provided group life, voluntary life insurance, short-term disability, long-term disability, and 401k.About BCforward:Founded in 1998 on the idea that industry leaders needed a professional service, and workforce management expert, to fuel the development and execution of core business and technology strategies, BCforward is a Black-owned firm providing unique solutions supporting value capture and digital product delivery needs for organizations around the world.
