Responsibilities
Job Summary / Purpose
Coordinates a system of direct case management to effectively follow patients throughout the continuum of care in a multiple health care delivery system. Role includes assessment of patient's status, evaluation, coordination of on-going needs, and planning of care and delivery of services, appropriate implementation of community resources through proper discharge planning functions, and monitoring of quality of service throughout the case management process. Performs direct Interqual/M&R utilization review on all acute admissions.

Graduate from accredited school of nursing.
Required Licensure and Certifications
Kentucky state licensure as a Registered Nurse.
Required Minimum Knowledge, Skills and Abilities
Prefer 2 years recent varied acute hospital experience. Strong clinical background with a broad range of clinical experience, and the ability to utilize data and regulatory information in decision making process. Adequate conversational English is required in order that the individual be able to take direction from management, understand how to complete job tasks, communicate adequately with patients and co-workers as necessary, understand and follow safety guidelines, and understand the Associate Handbook and all hospital policies.
PREFERRED Qualifications
Associates Degree in Nursing, CCM, Interqual, M&R, or Discharge Planning Certification. 3-5 years recent varied acute hospital experience, 1 year Case Management or home health working with Medicare/Medicaid and other federal guidelines or 2 years Utilization management in hospital setting. Also desirable, experience in discharge planning with case management model.
