Overview: Transition and Engagement Specialist - Community Transitions

This position will work with our multi-disciplinary team focused on moving individuals with severe and persistent mental illness and/or dual disorders wanting to transition from living in a nursing facility to community settings. This position will engage members interested in the Williams Comprehensive Transition Program with a person-centered and strength-based approach. Services will be provided in an array of settings including the community and nursing facility.

Core Responsibilities Include:

  • Outreach and engagement with residents currently residing at the nursing facility to determine interest in moving out;
  • Provide education and information to nursing facility residents, their families/natural supports and nursing facility staff regarding the Williams Consent Decree, services offered and Heritage services related to Williams Transition Program;
  • Obtain informed consent from class members and/or guardian interested in moving out and coordinate referrals for assessment;
  • Provide assistance to class members and agency teams to facilitate the successful and safe transition to living in the community. Including but not limited to: obtaining appropriate housing, mental health and psychiatric services, physical health services, public assistance benefits, resources for social or leisure activities, and developing connections assistance with natural supports. Provides direct assistance to help members move and obtain household items;
  • Assist individuals residing in the nursing facility in developing their skills including activities of daily living, harm reduction, medication management, healthcare practices, community access and accessing natural and social supports;
  • Assist in the development and implementation of class member's comprehensive service plan;
  • Work with the multi-disciplinary Comprehensive Transition team to assess, plan, and participate in provision of services;
  • Assist with coordinating and attend discharge staffing;
  • Complete all documentation and forms required to provide outreach and coordinate the transition process for Williams Class Members;
  • Complete all clinical documentation and required reports in an accurate and timely manner.

Knowledge, Skills and Abilities:

  • Ability to work with individuals with a mental health and/or substance use diagnosis;
  • Knowledge of community resources and/or ability to research/find;
  • Knowledge of healthcare resources, including psychiatric, substance use, medical, and/or other services;
  • Ability to meet people where they are at in their recovery, non-judgmental, and ability to build rapport quickly;
  • Ability to work in a team-based environment;
  • Ability to accept ambiguity, uncertainty, and change;
  • Ability to navigate electronic health records;
  • Ability to work collaboratively with the direct leaders and other clinical staff providing services to the person served
  • Ability to respond to supervisors, person served, and other clinical staff providing services to the person served.

Education and Experience:

  • Ability to function as a Mental Health Professional, which is defined as:
  1. 5 years' experience working with individuals in a behavioral health setting
  2. Certified Recovery Support Specialist (CRSS)
  3. A Bachelor's Degree in psychology, social work, human services or related field, or any other degree plus 2 years of experience.
  • Valid driver's license, reliable means of transportation and proof of current automobile insurance are required.

Salary Range: $47,840-$56,190 per year ($23.00 to $27.00 per hour) dependent on education and experience
