Job Description:
We are looking for a caring and patient Job Coaches. You will help adults with cognitive disabilities learn job skills and succeed at their jobs at a variety of worksites in the Antelope Valley.
Responsibilities:
- Teach real world job skills
- Support clients at their workplace
- Travel to various worksites in the AV in your vehicle
- Help clients solve work problems
- Encourage and motivate clients
- Observe and document workplace conditions and any issues encountered.
- Communicate effectively with employers
- High school diploma or GED
- Reliable transportation and Valid CA driver's license

- Ability to professionally represent our agency in the community

A. GENERAL RESPONSIBILITIES:
Provide consistent, effective on-the-job training, coaching and support for consumers working within community-based employment, as well as for those participating in a paid internship, in a manner that assists and supports them to achieve the desired results and expectations for themselves and for their employers.
B. SPECIFIC DUTIES ( essential functions are highlighted in bold print):
- Attend all consumer-related meetings, as directed (pre-employment meeting, semi-annual/annual review, orientation, special-called ID Team meetings, etc.).
- Review necessary paperwork to become familiar with consumer, employment site and position (Job Site Analysis, Worksite Assignment Form, Individual Habilitation Service Plan, Consumer Medical Information Record and Face Sheet).
- Obtain weekly schedule and report to sites as scheduled.
- Ensure consumer is adequately oriented to the worksite, job tasks, on-site supervisor and co-workers.
- Complete Task Analysis Form (during the consumer's first week on the job).
- Break down each task into its smallest component, step-by-step.
- Monitor the consumer's performance with regard to each task step and document the level of proficiency on the form.
- As the consumer becomes increasingly independent relative to each task, phase out the task analysis form.
- Provide training, support and guidance to consumers in the completion of their job tasks in a manner that ensures optimal performance in compliance with employer specifications, as well as individual safety and success.
- Provide guidance to the consumer in an orderly, purposeful and goal directed manner that is in keeping with the strengths and needs of the individual consumer.
- Seek to create natural supports for the consumer within the work environment.
- Seek to coach each consumer in the least intrusive manner possible and "phase out" as consumer becomes increasingly proficient on the job.
- Make accurate, thorough, and timely objective observations of consumer's abilities, behaviors, attitudes, and performance and document accordingly.
- Complete and submit paperwork in a thorough, accurate, and timely manner, as directed:
- Daily Case Notations
- Task Analysis
- Schedule Verification Log
- Timesheet
- 1-week, 60-days, semi-annual and annual progress evaluations
- Worksite supervisor evaluation (ensure completion by supervisor at community-based worksite)
- Accident/Incident Reports
- Establish and maintain rapport with consumer's community-based supervisor and interface with him/her as necessary and/or directed.
- Maintain confidentiality of consumer-related information at all times.
- Ensure professionalism at all times when interacting with other staff members or departments within DHE and at community-based placements.
- Serve as positive role model for staff and consumers through consistent compliance with ethical business practices and policies, guidelines and written procedure of Desert Haven Enterprises.
- Follow and comply with all safety rules and use good judgment concerning safe work behavior, cooperate with safety efforts, report potential hazards immediately.
- Promote cooperation and goodwill among outside agencies, business and community members by developing trust, building rapport, maintaining open lines of communication and exhibiting a commitment to excellence and ethical, professional business practices.
- Comply with DHE's Code of Ethics and other agency policies and procedures relative to business practices, interpersonal relationships and personal conduct.
- Advise supervisor of any and all communications and/or happenings, which are significant and/or unusual.
- Attend staff meetings, in-service training, seminars, etc. as directed.
- Perform other related duties as assigned.
- Minimum General Education: High school diploma, or equivalent; degree in related area preferred
- Specialized Education or Training: current certification in First Aid and CPR. Word processing experience, including use of Microsoft Word.
- Minimum Work Experience: One year experience in supervision; experience preferred in working with persons with developmental disabilities.
- Ability to effectively train and guide consumers in the completion of tasks, in accordance with employer specifications.
- Ability to oversee and coordinate individual work assignments.
- Ability to work effectively with persons with developmental disabilities.
- Ability to objectively observe and report consumers' individual strengths, needs, limitations and assets.
- Ability to establish and maintain effective and cooperative working relationships with employers and others.

- Ability to be flexible in covering allocated job coach hours in keeping with consumers' varied work schedules.
