Job Details

Hospital Case Management M-F days for either surgery, cardiac, GI, trauma

What you will be doing:

• **Patient Assessment:** Evaluate patients’ needs and develop comprehensive care plans to address medical, social, and emotional aspects of their health.
• **Care Coordination:** Facilitate communication and collaboration among healthcare providers, patients, and families to ensure seamless care transitions.
• **Resource Management:** Identify and arrange necessary resources, such as therapies, equipment, and community services, to support patient recovery.
• **Discharge Planning:** Prepare patients for discharge by providing education and resources, ensuring they understand their follow-up care.
• **Monitoring Outcomes:** Track patient progress and outcomes, adjusting care plans as needed to meet evolving needs.
• **Advocacy:** Advocate for patients’ rights and needs within the healthcare system, ensuring they receive appropriate and timely care.

Skills you need:

• RN license
• 2 years of in-patient RN experience is required
