Responsibilities:
The Public Education & Community Outreach Coordinator will engage partners to implement evidencebased or evidenced-informed strategies to promote the program, build public demand for cancer screening services, and identify eligible clients in priority populations, throughout the service region. In addition, the Public Education & Community Outreach Coordinator will ensure and coordinate implementation of client oriented, evidence-based screening interventions and strategies. The Public Education & Community Outreach Coordinator will use data to identify and locate eligible priority populations throughout the service region to target and prioritize public education, outreach and in-reach
efforts.
Public Education and Community Outreach Coordinator is responsible for planning and oversight of NYSDOH CSP outreach events, meetings and education sessions throughout 6 counties, Delaware, Otsego, Schoharie, Oneida, Herkimer and Madison. Responsible for all materials and supplies needed for event. Coordinators are responsible for developing partnerships with local, government, private and public health care facilities, stake holders and any other advocacy groups supporting the CSP program. Coordinator will oversee the collection of data required by NYSDOH for all reports and statistical data as well as what is requested by Program Coordinator. Coordinator will perform intake activities for clients in attempt to have screened. Will develop a network of medical care providers throughout the 6 counties service area to provide men and women a comprehensive, guideline-concordant breast, cervical and colorectal screening and diagnosis service and when necessary, ensure access to treatment. Will develop relationships with community organizations that provide resources to address barriers individuals may encounter during appointments and treatment.
Designs, implements and evaluates screening promotion and screening provision activities.
Provides strategies to work effectively with healthcare systems and other organizations to improve the implementation of activities.
Develops and implements a plan to regularly communicate with partners and providers about program services and operations. Such communication may be in writing, via phone, webinar and in-person meetings.
Engages partners to assess needs, conduct education, and develop, implement and evaluate comprehensive plans for outreach and in-reach recruitment activities to build demand for and provide screening services for eligible priority populations throughout the service region.
Uses data to identify and locate eligible priority populations throughout the service region to target and prioritize public education and outreach and in-reach efforts. It is expected that at least 75% of clients screened through the program will be aged 50 through 64.
Ensure implementation of effective strategies for educating members of priority populations about the importance of early detection and screening for breast, cervical and colorectal cancer
Ensure the delivery of clear and consistent messages about breast, cervical and colorectal cancer screening to increase the public demand for cancer screening and promote the availability of the local screening program.
Serves as the point of contact with community members, providers, partners and other organizations in the service region, providing education about the need for screening.
Collaborate with patient navigators, community health workers or other partners to provide one-on-one education to increase knowledge or influence attitudes and beliefs regarding the need for cancer screening.
Ensure collaboration with community partners to offer and/or provide group education sessions to community groups and organizations to provide education regarding the need for screening, intention to be screened, risk/benefits of screening and appropriate screening intervals.
Contacts identified patients regarding needed cancer screenings, providing patient education about the importance of cancer screening and assisting them to obtain screening appointments.
Coordinate education of local decision makers, community leaders and members of the public. Provide data, facts and client/personal stories for use by partners in these activities.
Coordinate partner and community participation in promotion and outreach activities promoting the need for screening.
Coordinate partner participation in promotion and outreach activities (e.g. Main Street’s Go Blue, cancer awareness month activities, other community events) as provided and directed by NYSDOH and Bassett Community Outreach Manager
Work with partners to enlist businesses and employers throughout the service region to promote cancer screening.
Recruit community programs working with cancer survivors to encourage survivors to be screened.
Ensure collaboration with existing chronic disease programs in the service region to conduct joint outreach and recruitment, and to promote clinical preventive services.
Ensure implementation of cancer screening and/or mobile mammography events to increase access to cancer screening, diagnosis and treatment services.
Performs similar or related duties as requested or directed.
Performs other duties as requested and observed by supervisor or manager.
Participates in NYSDOH sponsored data trainings as required or as directed by NYSDOH and the Program Coordinator
Coordinates and follows states suggestions of targeted out reach of particular clients
Required to meet states targeted goals of client requirement set forth in the monthly performance
measures.
Required to work a flexible schedule. Mornings, evenings and weekends
Coordinates, organizes and staffs education and outreach events.
Holds monthly education and outreach meetings
Provides reports to funding source including monthly, bi-monthly, quarterly and year end reports.
Assures accurate data collection and entry
Tracks Education and outreach activities monthly and reports to appropriate funding source as well as leadership
Seeks collaboration with local community members as well as stake holders
Facilitates effective communication with all collaborating parties to ensure sustainable outcomes that meet the needs of the program and all parties involved.
Ensures that there are sufficient numbers and types of providers in the network to meet the needs of the eligible population for comprehensive and timely cancer screenings and diagnosis.
Acts as first point of contact, assess eligibility, conducts client intake, and completes intake forms and other related tasks.
Refers clients in need of treatment for breast, cervical or colorectal cancer for enrollment in the Medicaid Cancer Treatment Program (MCTP). It is expected that 100% of eligible clients be enrolled in MCTP.
Develops relationships with community organizations that provide resources to address barriers individuals may encounter during diagnosis evaluation and treatment.
Ensures network providers are committed to treating men and women diagnosed with breast, cervical and colorectal cancer who do not qualify for MCTP.
Clearly communicates program eligibility guidelines to all providers in the network ensuring only eligible clients receive the program services.
Promptly communicate program changes to all contract agencies with the CPS program as well as all programs that refer cvlients to the program.
Ensure all providers submit all required forms, data and records in timely manner.

Works with advertising agencies throughout 6 county market to ensure program is reaching any and all potential clients
Qualifications:
Education:
2 year / Associate Degree, required
4 year / Bachelor's Degree, preferred
Experience:
Minimum 3 years experience in a healthcare setting, required
Minimum 5 years experience in a healthcare setting, preferred
