Job Purpose Summary: To provide assessment, evaluation, and therapeutic support to clients seeking mental health services. The Lead Intake and Assessment Therapist participates in multi- disciplinary team planning; clinical supervision; training in crisis management; assists with intake; and assumes administrative responsibilities in conjunction with Harmony Bay?s policies and procedures. The Lead Intake and Assessment Therapist acts as an interdepartmental liaison for linkage to supportive services for clients, utilizing support from external and internal entities. Core Functions: 1. Interdepartmental Liaison 2. Crisis Management 3. Training and Education 4. Linkage to Special Services 5. LMA Key Performance Indicators: ? 80 % of caseload appointments filled weekly. ? Notes are documented within EHR within 48 hours? time span (reviewed quarterly) ? 70% of eligible patients are linked to appropriate higher level of care and/or ancillary services (TMS, Spravato, IOP, nutrition, etc). ? Conducts Crisis Procedure trainings to demonstrate understanding of crisis management tools; Conduct ancillary service presentations for internal and external entities on TMS, Nutrition, Spravato, etc. ? Additional projects are completed within allocated deadlines to ensure maintenance of current programs and launching of new programs. Responsibilities Include: ? Performs evaluations and intake assessments which includes understanding of DSM-V diagnoses, treatment planning, input of various clinical and non-clinical information into electronic medical record, screenings, gathering of history, provisional diagnosing, narrative assessment, Releases of Information, etc. in a timely fashion. ? Assesses clients? needs, and communicates with all involved parties following HIPAA laws and regulations ? Collects data and comprises reports of specific crisis and intake data as assigned. ? Completes intakes for new clients seeking ancillary services (TMS, Spravato, etc). ? Arranges for follow-up appointments with appropriate medical (APN or Psychiatrist) and/or clinical (Therapist) staff member. ? Constructs APN Online Scheduling with PAC Manager and oversee caseload distribution. ? Distribution of new clients onto necessary provider caseloads. ? Oversees and assists with management of Therapy Waitlist. ? Identifies caseload criteria for new and existing providers based on credentials and employment type (F/T, P/T, FFS, RVU). ? Assists with pre-book process for new providers. ? Ensures all necessary paperwork and documentation are forwarded to appropriate staff. ? Prepares progress notes and/or crisis intervention notes and documents all client telephonic/face-to-face contacts, as well as consults with other clinicians. ? Maintains all other clinical records as required. ? Maintains knowledge of Electronic Medical Record system and changes. ? Constructs amendments to Intake Paperwork packet in accordance with HIPAA laws & regulations, and HBW policies and procedures. ? Collaborates with other departments to ensure collection and completion of client Intake Paperwork packet. ? Provides psycho-therapeutic counseling for caseload, weekly. ? Flags clients for potential eligibility regarding ancillary services, such as TMS, MH IOP, SUD IOP, etc. ? Documents clinical and medical eligibility criteria for ancillary services. ? Executes the Refer Out Process for clients not meeting clinical or medical criteria for Harmony Bay Wellness. ? Provides guidance on Refer Out Process and LOC eligibility to Intake Department. ? Ensures timely completion of all Internal and State Incident Reporting within the Intake Department. ? Reviews intake documents completed by Intake Clinicians and other clinical staff . ? Oversees the collection of discharge paperwork documentation for incoming clients. ? Supports Intake Clinicians in documentation review and/or administrative discharge of clients not adhering to 30-day Discharge Paperwork Policy. ? Provides monthly supervision to Intake Clinicians. ? Facilitates weekly Intake Department meeting. ? Acts as an Interdepartmental liaison to ensure execution of all Clinical and Business Development functions within the Intake Process. ? Acts as a Clinical contact to provide consultation to Client Care Manager(s). ? Performs and scores necessary assessment scales (PHQ9, MADRS, etc) for clients seeking ancillary services. ? Assists with rotating on-call schedule for crisis management. ? Acts as Hiring Manager for Intake Department. ? Trains onboarding Intake Clinicians on all job functions and HBW policies/procedures. ? Trains onboarding, non-clinical staff, on HBW crisis procedures. ? Provides education on ancillary services to onboarding and current staff members, across departments. ? Provides presentations on HBW and ancillary services to other Health Networks. Job Type: Full-time Required education: Master?s Degree, NJ LCSW, LMFT or LPC Licensure Required experience: 2-4 years? experience working in Clinical Treatment environment Proper Knowledge and Skills: Core Competencies: ?
