Position Summary: The Community Health Navigator works in collaboration with all members of the health care team. Focuses primarily on supporting individuals recently served in the SVMC Emergency Department. Utilizes appropriate screening tools to identify needs and link patient to services. Develops care initiatives in partnership with team members. Accepts referrals for the navigation of individuals in recovery to services, resources, social supports (i.e.-transportation, food, housing, employment, etc), and or other community resources as appropriate.. Serves as an advocate and assists in identification and improvement of service delivery. This position requires expertise in care coordination, case management, knowledge of social determinants of health.

  • Collaborate with practice and health care teams
  • Outreaches individuals recently served by the SVMC Emergency Department
  • Conducts relevant screenings.
  • Supports provider treatment plan and promotes adherence to physician/provider recommendations and instructions.
  • Serve as a community referral resource within the practices..
  • Identify service delivery problems and potential for effective management intervention.
  • Maintain annual competencies for role.
  • Document all communication with individuals in Athena.
  • Work at different office locations as needed.
  • Perform all other duties as assigned.

Education and Experience :
  • Bachelors' degree in psychology, social work or related field preferred
  • One years case management experience in a community, hospital or medical/behavioral health setting preferred. Knowledge of a variety of local, state, and federal resources necessary.
  • Experience with the utilization of screening tools preferred
