JOB SUMMARY

The mission of the Occupational Health Care Navigator is to facilitate and ensure continuity in applicant and employee care and serve as a conduit of care between the client occupational health service and the community provider network. The OH Care Navigator role will expertly and efficiently coordinate occupational health care services for identified candidates and employees on behalf of client. The OH Care Navigator role is supported by and reports to the Clinical Operations Manager. The individual will work closely with the Client Relations, Clinical Operations team, Network Providers, and Client HR Team.


ESSENTIAL DUTIES AND RESPONSIBILITIES

Coordination of Care

  • Coordinate candidate and employee care services within provider network, imaging centers, labs, community resources, etc. to ensure seamless care delivery and satisfaction.
  • Ensure medical records are provided to Proactive MD following any visit and that records are sent to the employer. Review client case progress and determine case closure.
  • Send notification to employer/network provider regarding procedures and ensure completion of scheduled health care services.
  • Assist client/ employees in navigating the healthcare system.
  • Coordinate Care and Case Management of Occupational Health Services.
  • Demonstrate exemplary customer service in coordination and communication surrounding Health Center visits.
  • Work in coordination with the Client Operations Manager and Client representative to develop employer Occupational Health case management plan.
  • Coordinate patient care and Occupational Health services.
  • Be knowledgeable about and provide information to employer/patients regarding Occupational Health services.
Health Center Synergy
  • Encourage collaboration and effective communication, with provider network, through service and professionalism.
  • Build and maintain strong relationships with Provider Network and collaborate with Data Analytics to demonstrate high-quality outcomes.
  • Prepare and participate in Proactive MD meetings to ensure excellent cross communication and collaboration.

REQUIRED KNOWLEDGE, SKILLS, & ABILITIES

Required:

  • 2+ years of experience working in Occupational healthcare setting, Medical or Healthcare Social Work in similar setting, or Case Management role
  • Knowledge and understanding of OSHA medical surveillance requirements and process
  • Current BLS certification through American Heart Association valid for at least 90 days after start date
  • Medical Terminology, including knowledge of the concepts and principles of medical case management and medical terminology, including understanding of specialists
  • Ability to communicate effectively and maintain working relationships with people from diverse backgrounds
  • Knowledge of the healthcare industry and available patient resources
  • Ability to prioritize needs and plan work accordingly
  • Knowledge of HIPAA confidentiality requirement

Preferred:
  • Bachelor’s degree in social work or Registered Nurse in good standing with board of nursing
  • Experience with Occupational Health Case Management
  • Knowledge of third-party payer requirements
  • Licensure in LPCW, LCIS, LPC or similar


WORK ENVIRONMENT & PHYSICAL REQUIREMENTS

This job primarily operates in a hybrid clinical environment and is required to interact with patients/employers for the majority of the day. The physical demands to perform the essential functions of this job are: •

  • Effective communication, mobility, ability to operate office equipment and travel.
  • Must be able to remain in a stationary position at their work area for prolonged periods of time.
  • Additionally, the employee will need to be able to lift/move/store individual 10–15-pound boxes and items such as office and medical supplies and equipment.


POSITION TYPE & EXPECTED HOURS OF WORK
Typical workdays are Monday through Friday, 8:00am to 5:00pm. This role is considered an exempt position. Evening and weekend work are infrequent but may occasionally be required as business needs dictate. This is a management role with professional growth opportunities within Proactive MD.
TRAVEL
Infrequent, domestic travel may be required and should be expected to be less than 15% of the position’s overall responsibilities.
