The Psychiatric Rehabilitation Service Coordinator (PRSC) is responsible for coordinating the overall delivery of care to each individual within his/her caseload. The PRSC is responsible for helping individuals address their mental health challenges enabling each to function at his/her highest practical level.
Essential Job Functions:

  • Provides the resident with a stable therapeutic relationship.
  • Welcomes the new resident and orients them with the facility.
  • Reviews and assists the resident in understanding the treatment plan arid program schedule.
  • Provides and/or coordinates the delivery of the psychiatric rehabilitation services program.
  • Accurately and timely completes assigned of the resident's comprehensive assessment.
  • Re-evaluates the resident on a quarterly basis and documents this re-evaluation.
  • Oversees the interdisciplinary team in the development of the individual treatment plan.
  • Documents the resident's progress on a regular basis, including incidental and quarterly notes.
  • Contacts community agencies for the purpose of resident referral.
  • Assists the resident to participate in individual and group programs and to utilize these programs to full advantage.
  • Additional duties as assigned
Requirements
  • Bachelors or Master's degree in a human service progression (i.e., Social Work, Psychology, Sociology, Counseling Psychology)
  • One (1) year of supervised experience in mental health or human services
  • Demonstrated skills and ability in working with and understanding the needs of the residents, families, staff members and personnel from community agencies
