We have spent over 40 years advocating for and creating a health-home for the many that have been denied access to care. As health care activists, we are dedicated to ensuring that individuals who do not have healthcare coverage receive the same quality healthcare as those who do. From our genesis as a community health center in 1971, we understood that we cannot heal our communities without also addressing the economic and social factors that affect health. Many times, this requires that we go outside and provide services in other settings that link individuals to our health center for preventive and primary care. Over 90,000 individuals come to La Clínica because it is a welcoming place that addresses the whole person, coordinating and connecting them to a broad network of services to improve and maintain their health and well-being. While we are still known for our activism and spirit of social justice, we are also proud to have grown into a sophisticated provider of primary health care services with 35 sites across Alameda, Contra Costa and Solano counties.
Who you are : Under general direction, the individual in this position is responsible for developing, coordinating, and evaluating projects and health promoter related programs, in the Preventive Medicine section of the Health Education Department. This individual will also provide primary health education classes on topics which include diabetes, pre-diabetes, hypertension, asthma, smoking cessation, cholesterol, and weight management.
Major Areas of Responsibility include but are not limited to:
Who you are : Under general direction, the individual in this position is responsible for developing, coordinating, and evaluating projects and health promoter related programs, in the Preventive Medicine section of the Health Education Department. This individual will also provide primary health education classes on topics which include diabetes, pre-diabetes, hypertension, asthma, smoking cessation, cholesterol, and weight management.
Major Areas of Responsibility include but are not limited to:
- Coordinates the planning and implementation of special projects and grants related to chronic disease prevention, health education, health promoter-related projects, and research grants.
- Facilitates regular team meetings with multidisciplinary staff, related to project coordination, evaluation and quality assurance.
- Plans, recruits, trains, coordinates and oversees activities of the health promoters who work in the clinical areas.
- Facilitates the communication between providers and patients, clinical health educators and health promoters.
- Coordinates the scheduling of health education classes and facilitators at Transit Village.
- Monitors and evaluates projects, conducts chart reviews and oversees data collection.
- Responsible for maintaining accurate program records and tracking project budgets.
- Writes and submits reports in accurate and timely fashion, as required by funders.
- Conducts one-on-one and group education related to chronic disease prevention and self management, on topics including diabetes, pre-diabetes, cholesterol, hypertension, obesity prevention, and smoking cessation.
- Maintains documentation by charting SOAP notes in the progress notes.
- Works with community organizations; attends meetings; advises groups in health activities.
- Evaluates and develops health education materials on a variety of topics.
- Other duties as assigned by supervisor.
- Requires graduation from college and the possession of a Masters degree in public health with specialization in health education or related area, with at least two years experience working in health education.
- Must hold a current BLS (Basic Life Support) Certificate. You will have 30-day grace period to obtain your certificate from the date of hire.
- Knowledge of principles, methods and materials used in health education.
- Experience in program evaluation design, implementation and oversight.
- Strong group facilitation, presentation and organizational skills essential. Ability to facilitate meetings in English and Spanish.
- Experience working with chronic diseases, especially diabetes and its management preferred
- Knowledge of contemporary principles and practices of nutrition, nutritional value of foods, interpretation and implementation of special diets, and meal planning and preparation that is culturally appropriate.
- Ability to effectively engage with patients on a one-to-one basis as well as in a group.
- Ability to work well independently as well as in a team.
- Excellent organizational, administrative, interpersonal, verbal and written communication skills.
- Creative problem-solving skills, and ability to exercise sound judgment in quickly evaluating situations and rendering prompt decisions.
- Ability to work with community groups and multidisciplinary staff in developing and implementing programs, and to develop and maintain cooperative relationships with lay and professional persons.
- Ability to work across cultures and demonstrate support of diversity, equity and inclusion.
- Must have flexible schedule to conduct classes and attend community events after regular business hours and on weekends, when necessary.
- Bilingual; bi-literate in English / Spanish required.
