Duties:
- Performs all duties in accordance with Westminster Village values, acting with love and respect in all we do, do the right thing, and go above and beyond.
- Develops and maintains team and client schedules.
- Works to resolve client and team concerns.
- Orients new team members to department operations and policies.
- Provides guidance and recommendations for additional training as needed.
- Overseeing of the Westminster Village Home Care Program by supervising Home Service Workers.
- Provide skilled-nursing visits for clients when necessary.
- Manages the reliability of client and caregiving information in Westminster Village's numerous?Information Management systems.
- When the need arises, provide clients with service and assistance, as outlined in a predetermined service agreement.
- Vigilantly communicates, particularly when changes are observed in clients' condition with caregivers, clients, and/or their family members to foster strong working relationships and client outcomes. Facilitates the successful exchange of communication by serving as the primary liaison between clients, caregivers, family members, guardians, and other members of the Home Care team.
- Completes documentation and all other necessary paperwork in a timely manner in accordance with department policies and procedures.
- Attends in-services, meetings, and educational training in accordance with Westminster Village policies and procedures.
- Perform initial evaluations for Home Service clients.
- Valid driver's license, reliable access to a car, and active car insurance
- Successful passing of a drug test and IDPH fingerprint criminal background test
- Valid Illinois Registered Nurse License
- Acute Care Nursing – 1 year minimum preferred
- Home Health/Care – 1 year minimum preferred
- BLS required
