Turning Point Community Programs is seeking an Assessment Clinician for our Providence Center Program in Grass Valley. This position is eligible for a $3000 sign-on bonus. GENERAL PURPOSE
Under the administrative supervision of the Clinical Director or Program Director, this position is responsible for providing psychosocial, rehabilitation and recovery-oriented services for adults.
DISTINGUISHING CHARACTERISTICS
This is an at-will direct service position within a program. The position is responsible for ensuring that clients are introduced into the Program in a comfortable, thorough and timely manner. Completes the initial assessment of clients to confirm eligibility under target population; keeps clients and families engaged in services until assignment to primary Behavioral Health Specialist is successful.
ESSENTIAL DUTIES AND RESPONSIBILITIES - (ILLUSTRATIVE ONLY)

  • Responsible for completing the intake assessment and initial registration paperwork for new members, in the field or in the office.
  • Responsible for supporting the development of the Individual Client Plan in collaboration with the member and the treatment team through linkage and handoff at Team Meeting.
  • Identifies strengths and challenges of member and their family/identified support system
  • Screens for barriers to treatment and identifies member's natural supports
  • Interacts productively with the treatment team in order to provide holistic assessment and engagement services that are member driven and recovery oriented.
  • Completes all required annual paperwork as assigned in a timely manner.
  • Supports Clinical Director to ensure annual chart efficiency and compliance with Medi-Cal standards
  • Provides prompt intervention in the event of a crisis both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis situation.
  • Provides "on-the-spot" counseling that is both helpful to the members and consistent with the philosophy of the program.
  • Initiate contact with previous provider, Primary Care Physician, Psychiatric Hospital and/or collateral resources, to obtain assessment information and provide case management during transition as necessary.
  • Ensure member's discharging from psychiatric hospital are linked to agency Psychiatrist within 30 days. Ensure member's transferring to services with medication already in place are linked to agency psychiatrist.
  • Develop initial safety plan and monitor safety planning in high risk cases during assessment period.
  • Complete Service Requests and links members and their families/identified support systems to community resources as appropriate.
  • Facilitate groups and provide individual and family therapy.
  • Supports Family Advocate with engagement of new referrals to program.
Schedule: Monday - Friday 8:30 am - 5:00 pm (various shifts available, full-time or part-time)
Compensation: $34.00 - $36.08 per hour

Position Requirements
MINIMUM QUALIFICATIONS AND CERTIFICATIONS & LICENSES
Education, Training and Experience:
A typical way of obtaining the knowledge, skills and abilities outlined above is through graduation from an accredited college or university with a master's degree in Social Work or related field; a minimum of 2 years' experience as a provider of mental health services, and valid registration with the Board of Behavioral Sciences (BBS).
Licenses; Certificates; Special Requirements:
  • Valid registration with the Board of Behavioral Sciences (BBS)
  • California driver's license & current vehicle insurance/registration
  • Reliable means of transportation capable of passing vehicle safety inspection if more than five years old excluding all modes of two-wheeled transport inclusive of bicycles, mopeds and motorcycles
