Southwest Behavioral & Health Services is seeking a Jail Navigator to join our Community Resilience Program! This position will give you the opportunity to make an immediate and meaningful impact in the lives of our members we serve.
In this position you will be supporting members in Phoenix, Arizona at our Criminal Justice Services community with maintaining and improving their physical and mental health . This position will report to the Program Coordinator who will support you and your team's success. The work schedule will be Monday through Friday with a focus on flexibility and work-life balance.
Ideal Candidate

About the Position
The Direct Care Provider delivers 1:1 or group services in a home setting, providing support in maintaining a safe living environment, assisting with self-administration of medications, coordinating with other providers, developing socialization opportunities, and increasing connections with the community. You will be part of a phenomenal team, working with leadership that will provide you with weekly 1:1 supervision, bi-weekly team meetings, and monthly clinical oversight. Each day is a unique experience! You could spend your morning helping members keep their living environment safe and healthy and your afternoon bowling or hiking. Or it is possible that you may take members to the library in the morning and spend your afternoon teaching symptom management skills to improve quality of life. Each site offers unique programming based on the skills and passions of our Direct Care Providers .
Duties & Responsibilities
  • Position works as a Peer Navigator serving the State Targeted Response Grant for Outreach and referral into Opioid and Medication Assisted Treatment programs.
  • Provides extensive community outreach work based on the needs of the team and individuals served in multiple locations.
  • Works well independently in the community managing time and appointments as required by the program and service needs. Meets and balances multiple demands.
  • Develops working rapport with Persons Served as a priority of service
  • Acts as a Navigator and Case Manager, brokering, planning, and referring persons into Medication Assisted Treatment and other community services using an integrated approach.
  • Provides interim short-term services as need to facilitate transition
  • Is familiar with various funding mechanisms as to enroll the client in services and benefits in accordance with needs.
  • Completes comprehensive assessments, intake and enrollment paperwork in a timely manner
  • Participates in and writes Integrated Treatment planning and interventions.
  • May work to give input into pre-release planning in the jail or court system
Minimum Requirements
  • Minimum requirement consists of a 22-year age
  • A High School Diploma/GED and two years of relevant experience; OR
  • A related Associate's degree and six weeks of relevant experience; OR
  • A non-related Associate's degree and one year of relevant experience; OR
  • A related bachelor's degree; OR
  • A non-related bachelor's degree plus six months of relevant experience in behavioral health
  • Peer Certified preferred
  • Knowledge of Opioid Addiction personal or professional preferred
  • Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive within eight (8) weeks of hire (as required per program requirements).
  • Required to maintain a valid Arizona driver's license and pass motor vehicle screening for insurance purposes in order to transport clients, based upon need/requirements of the clinic and/or program
