As the Rehabilitation Hospital Case Manager, you will collaborate with departmental staff to identify barriers to care and develop solutions, complete documentation per workflow requirements, communicate with caregivers, and coordinate patient care team conferences to achieve identified patient goals and outcomes.
The ideal candidate will have a Masters or Bachelors in Social Work or related field, certification in Case Management or Rehabilitation Nursing, and a minimum of 2 years of experience in an inpatient setting.
