This position is open to the public and all current City employees covered under the City’s collective bargaining agreement with AMERICAN FEDERATION OF STATE, COUNTY, AND MUNICIPAL EMPLOYEES (AFSCME) – BARGAINING UNIT 1, 3, 4 & 5. Only employees in City job titles in this bargaining unit are eligible to bid.

ESSENTIAL DUTIES
As an essential member of the Chicago Department of Public Health – Immunization Services Division, the Clerk III will perform clerical duties at mobile vaccination locations throughout the City of Chicago.
Provides clerical support at mobile vaccination sites throughout the City of Chicago, which includes arriving to site half an hour before the start time to assist with clinic set-up
Registers patients, efficiently, accurately, and expeditiously, inputting and/or updating all pertinent information, including demographic information to complete electronic/paper registration
Performs look-up for eligibility status within IL HFS database, and MEDI
Performs look-up for immunization records in the State Immunization Registry (I-CARE) and records all doses of vaccines given to the patient in the registry system and/or electronic medical system
Accepts requests and provides copies of clients records according to CDPH and HIPAA consent requirements.
Screens clients’ immunization history for vaccines needed and provides vaccine information statements
Transports biological, sharps, and other required supplies to mobile vaccination sites in a safe and secure manner
Assists with outreach activities such as calling schools and daycares during low immunization activities
Assists the Public Health Nurse I with weekly and monthly reports, monthly inventory of vaccines, and clinic supplies
Responds to public inquiries about CDPH Immunization Program services
Additional duties may be required for this position

Qualifications:
MINIMUM QUALIFICATIONS
Experience
· One year of clerical experience, or an equivalent combination of training and experience
PREFERRED QUALIFICATIONS
• Previous work experience performing clerical duties.
• Previous work experience recording and verifying data for accuracy.
• Bilingual Spanish and English
• Previous work experience with medical records
• Previous work experience in healthcare settings
• Associates Degree from an Accredited School or University
• Proficiency with Microsoft Office Suite
• Must have a vehicle and a valid driver's license
SELECTION REQUIREMENTS
Interview: (In person or MS Teams), testing: exercise to be completed before interview, etc.
