Overview: As a Hospice RN Liaison, you will have the opportunity to educate healthcare professionals about hospice services and provide end-of-life solutions committed to clinical excellence, compassion, and dignity. We offer a sign-on bonus and extensive benefits for eligible candidates.
Responsibilities:
- Ensure effective and timely intake and referral processes to meet patient and family needs
- Complete the admissions process for referred patients, including emergency cases
- Provide accurate and timely information regarding hospice services to inquiring parties
- Act as a patient advocate in evaluating and developing care plans
- Call on various account types and offer in-services and education on the benefits of hospice
Requirements:
- Compassionate and empathetic RN with a hospice heart
- Active RN license in the relevant state(s)
- Minimum 2 years of healthcare experience
- Established and respected within the community
