Stepwell Care is a new outpatient substance use disorder provider providing in-person and telehealth care. We are looking for a dynamic and experienced counselor to join as an early member of our team.

The Recovery Specialist is responsible for providing comprehensive assessment, treatment and care to individuals with substance use disorder at all outpatient levels of care.
Utilizing evidence-based practice the Substance Use Recovery Specialist will work with therapists and counselors to assist clients on their recovery journey. Substance Use Recovery Specialists conduct assessments, provide support services, facilitate group sessions, and collaborate with a multi-disciplinary team to ensure the highest quality of care. This role requires a deep commitment to patient-centered care, continuous improvement, and the utilization of technology to enhance service delivery.

Duties and Responsibilities:
- Uphold Stepwell Care values and care principles.
- Enable safe and timely admission by supporting the completion of a comprehensive biopsychosocial screening and assessment using ASAM dimensions (fourth edition).
- Identify the need for urgent escalation or referral.
- Undertake initial patient intake including recording of vital signs and CLIA waived point of care testing within training and competencies.
- Work with the multidisciplinary team to ensure holistic assessment of all ASAM dimensions.
- Provide clients and families with education on overdose prevention.
- Document screening and assessment in line with procedures and training, making effective use of technology.
- Provide evidence based care, treatment, case management and education in support of identified client needs, preferences and goals.
- Facilitate in-person and telehealth process, education and skills groups.
- Provide case management and referrals to support identified needs and goals.
- Ensure timely referral where needed to meet identified needs or goals.
- Utilize the Stepwell Care App to engage patients in education and treatment planning.
- Participate in the creation of education and development of referral pathways to support patient goals.
- Support coordination of client transportation, where necessary traveling with clients in third party transport to support their care.
- Actively engage in staff meetings, multidisciplinary treatment team meetings and communicate effectively to enable safe effective treatment.
- Document attendance, assessment, treatment notes and plans in line with procedures and training, making effective use of technology.
- Support UR and billing through timely documentation and responding to requests for further information or documentation.
- Actively engage in the culture of learning and innovation at Stepwell Care.
- Maintain education, training and competencies as required for the role.
- Engage proactively in supervision, performance review and personal development.
- Report all grievances, incidents and near misses.
- Engage in quality improvement activities including staff surveys, data collection, audits investigations and improvement activities.
- Proactively provide constructive feedback on policies, procedures, processes and systems.
- Respond to and report grievances, incidents including AMAs, near misses and emergencies within training and competencies, escalating as required.
- Effectively use technology including the Electronic Medical Record and telehealth to deliver and document care.
- Maintain client confidentiality through appropriate use of technology and compliance with policies, procedures and processes.
- Maintain client rights and ensure adherence to client responsibilities.
- Maintain a clean and safe environment of care.
- Respond to, resolve and report any environment of care risks.
- Comply with infection control policies and procedures.
- Maintain current knowledge of and work within all Stepwell Care policies, procedures and processes.
- Other duties as assigned.
- Knowledge of substance use disorders, mental health and co-occurring complications.
- Awareness of risk assessment tools and patient outcome measures.
- Competent and empathetic communicator.
- Ability to network and collaborate with healthcare and other social agencies and non-profit organizations that can provide services to clients and families.
- Works proactively to solve problems in support of clients safety and outcomes.
- Knowledge of laws and regulations for the treatment of substance use disorders.
- Flexibility to adapt to schedule changes.
Required
- High School Diploma or General Equivalency Degree.
- Certified or willingness to complete within 12 months of appointment any of Recovery Support Specialist (CRSS), Certified Recovery Peer Specialist (CRPS), Certified Addiction Counselor (CAC) .
- A minimum of six months of mental health, substance use or related work.
- Meet the required standards for or have a current Level 2 Background Screen.
- 1 year of mental health, substance use, or related work experience.
