POSITION SUMMARY
The purpose of your job position is to assist in the planning, developing, implementing and evaluating the social services programs to assure they meet the emotional and social needs of the residents in accordance with current federal, state and local standards that govern the facility, and as directed by the Administrator.
The Director Social Services Interim will treat each resident fairly, kindly, and show them dignity and respect. They will knock on the residents door before entering. They will greet them upon entering their room. They will refer to each resident by their name. They will communicate clearly, empathetically, and effectively when speaking to residents, family members, visitors, all staff and governmental agencies. They will work cooperatively with all departments and multidisciplinary teams. They will demonstrate patience, initiative and willingness to assist residents that may be difficult. They will relate all pertinent information concerning a resident's condition to a charge nurse when required. They will be committed to always doing the right thing.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Complete all required social services charting, including Minimum Data Set (MDS), psychosocial evaluations, social histories, assessments, quarterlies, updates as needed and Patient Care Plan (PCP) entries to meet federal/state licensing requirements, facility policies and procedures and to accurately reflect Resident needs
Discharging, appointment setting and arranging transportation for residents
Managing, scheduling, mentoring and hiring staff
Adhering to the facilities operational budget
Participate as a member of the Interdisciplinary Team (IDT)
Participating in facility surveys
Compiling and maintaining the social service directory listing
Assisting the QA Committee in appropriate plans of correction if warranted
Keeping updated with news laws and regulations as they pertain to resident care
Following Infection and Control policies and Safety and Sanitation procedures
Reporting any occupational exposures to blood, body fluids, or other hazardous materials immediately
Protecting each residents file and data with confidentially per HIPAA standards
Abiding with all facility policies and procedures including not disclosing user ID codes and passwords

REQUIREMENTS
Education / Licensure
Bachelor's Degree in Social Work or a bachelor's degree in a human services field including but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology; and one year supervised social work experience in a health care setting working directly with individuals required for facility with more than 120 beds.
Bachelor's Degree in Social Work or Social Service Designee Certification with at least 1 year experience for facilities under 120 beds.
Qualifications / Experience
Minimum of 1 year of experience in social services skilled nursing setting
Minimum of 1 year of supervisory experience in skilled nursing
Excellent English language usage in writing, speaking and presenting
Proficiency with Microsoft Word, Excel, and PowerPoint
Knowledge of Point Click Care a plus
Working Conditions
May encounter frequent interruptions
May be involved with residents, family and government agencies
May be requested to work beyond scheduled working hours at times
Is subject to call back during emergency conditions (e.g severe weather, evacuation, post disaster, etc.)
May be subjected to odors, dust, disinfectants, tobacco smoking or air contaminants
May be exposed to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses
Physical Requirements
Must be able to move intermittently throughout the day
Must be able to push, pull, move and or lift up to 25 lbs. to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 15 feet
