Project Healthy Grandparents (PHG) is dedicated to enhancing the well-being of families where grandparents are raising their grandchildren in parent-absent households. Since its inception in 1995, PHG has positively impacted over 3,000 individuals by providing comprehensive services such as home visits, parenting education, legal referrals, early intervention, and transportation assistance.
Project Healthy Grandparents (PHG), a program operating within the Byrdine F. Lewis College of Nursing and Health Professions at Georgia State University since 1995, is actively seeking a Sponsored Funded Social Worker II , working as a Youth Services & Community Outreach Coordinator, to join our team.

YOUR ROLE AS Sponsored Funded Social Worker II:
In this role, the Sponsored Funded Social Worker II, under the title of Youth Service & Community Outreach Coordinator, plays a crucial role in fostering relationships and creating opportunities for youth, particularly those in grandparent-headed families. Responsibilities include overseeing youth activities, identifying and partnering with relevant service providers, and facilitating referrals to meet the specific needs of youth. The coordinator will also manage community outreach efforts by identifying partner organizations, promoting available services, and coordinating referrals for short-term case management. Furthermore, the role encompasses tracking outcomes, preparing reports, and collaborating with an interdisciplinary team to provide comprehensive support for the families served. This position is vital for enhancing community engagement and improving the well-being of youth in the area.
KEY DUTIES AND RESPONSIBILITIES:
- Assess youth service needs for each family.
- Identify community partners, schools, and supportive services to assist youth and grandfamilies in the short-term case management project.
- Coordinate referrals for youth services and short-term case management, transportation, and collaborate with interdisciplinary teams and community partners to assist families with their needs and goals.
- Create and organize PHG's youth activities, workshops, groups, and events.
- Attend community events and activities.
- Supervise educational support Graduate Research Assistants (GRAs).
- Participate in youth service meetings and coalitions.
- Prepare and maintain documentation, including monthly service reports and progress notes on all services utilized by families, using database and spreadsheet programs.
- Attend departmental meetings and work on the development and implementation of policies and procedures, along with other responsibilities as assigned by the Assistant Director.
SPONSOR FUNDED:
***This is a regular, full-time position which involves on-campus and online duties, with job continuation dependent upon grant funding. Some evening and weekend work will be required occasionally based on clients' needs.
