DUTIES AND RESPONSIBILITIES:
- Meets newly enrolled individual's after screening is reviewed, to assist with enrollment process and begin to develop therapeutic rapport.
- As part of the Integrated Team, assists individual's with identifying meaningful goals for their Individual Action Plan/Treatment Plan focusing on the following areas: mental health recovery education, daily living skills, recovery from additions, relationships, vocational & leisure.
- In conjunction with the individual and Integrated team develops a comprehensive Individualized Action Plan (IAP)/Treatment, per Program Regulation. Ensures a multidiscipline approach is taken, using evidence-based practices which support recovery.
- Practices a trauma informed approach to treatment planning and service delivery.
- Work with individuals through life transitions and collaborates with individual and Integrated Team to develop the Crisis Management Plan.
- Integrates the PRISMModel® for effective treatment of serious and persistent mental illness, into everyday practice and clinical intervention.
- Promotes integrated services that meet the complex needs of individual's recovery goals
- Develops interventions that encompass all the individual's life domains including but not limited to the following:
- Physical and psychological wellness
- Activities of daily living
- Safe Housing
- Interpersonal skills and relationship building
- Employment and community connectedness
- Cultural and spiritual
- Arts and Wellness
- Facilitates peer lead activities, empowering enrolled individuals to take a lead in the planning and implementation process
- Develops, in conjunction with peer lead groups, information related to wellness and recovery
- Engages in non-judgmental, compassionate active listening and shares personal recovery experiences to build peer relationships
- Provides peer counseling, support and education to validate individual's' life experiences.
- Works with individuals to identify and utilize preferred recovery tools and self-care techniques (i.e. WRAP, peer groups).
- Utilizes motivational interviewing techniques.
- Encourages individuals use of and involvement in the Recovery Learning Community, where applicable.
- Supports participation in individual's self-help & advocacy organizations that promote recovery.
- Provides training and consultation to team to facilitate understanding individual's' perspective and protection of individuals rights.
- Peer encourages on-going engagement and involvement of family members in all phases of service delivery and evaluation based on the desires of the individual this may include ensuring family members understand:
- (1) the individual's illness/addiction
- (2) the service delivery model
- (3) the roles and functions of the Individual's Team members/Intensive GLE staff and how to contact them
- (4) how the family will be supported. As agreed upon with individual develops communication plans with family members.
- Maintains clinical documentation/records that is clear and concise, reflecting individual's preferences, needs and recovery and in compliance with FHR/ DMH policies and procedures as well as CARF standards
- High School Equivalency required, AA or BA in Human Service field preferred.
- Must possess lived experience (defined as having suffered from an addiction) and one Year of addictions recovery is required. Crisis Intervention experience preferred.
- Demonstrated strong time management and organizational skills
- Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions
- Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication
- Must have current, valid driver's license in state of residence, three (3) years driving experience in the United States, and acceptable driving record
