- DentalDegree with appropriate licensure to practice in Indiana
- Musthave completed an accredited Advanced Education in General Dentistry or GeneralPractice Residency program
- Atleast 1 year of experience in not-for profit environment (not required buthighly preferred)
- BLSCertification
