Summary of Position
Reporting to the Seattle Community Safety Initiative (SCSI)Resource Navigation Manager and supported by Resource Navigation Lead, the Resource Navigator partners with the Community Safety Team to form a Care Team that provides intensive wraparound services to participants through relationship building, coaching/mentoring, safety planning, strength and need assessments, goal planning, coordination of community services, and systems navigation and advocacy. Care Team caseloads should be between 10-15 participants. Participants may be supported for up to 18 months.
Essential Responsibilities
  • Intake, Assessment, and Goal Setting
    • In coordination with the Community Safety Team, provide one-on-one outreach to youth and young adults between the ages of 12 to 30 who meet three risk factors and/or those referred through SCSI and RPKC network. Risk factors include:
      • Regularly singled out by law enforcement, even if no arrest
      • Has been shot and/or seriously injured from violence
      • Has had a close peer, friend, or family member shot and/or killed in the last 3 years
      • On probation or parole
      • Lives or hangs out in hotspot areas
      • History and/or immediate risk for engagement in gun-involved activity
    • Connect with participants and establish a supportive relationship
    • With Community Safety Team Member, maintain consistent contact - including phone calls, texts, and in-person meetings-with each person on their caseload in accordance with their phase:
    • Review service plans with participants every 60 days and edit service plans as needed. Address what barriers have been keeping them from achieving their goals. Celebrate each accomplishment!
  • Program Support
    • Connect participants to comprehensive wrap-around services such as housing, employment, medical support, mental health, substance use, legal advocacy, etc.
    • Schedule appointments for participants and work with Community Safety Team Members to coordinate transportation as needed
    • Develop and maintain relationships with systems players with whom participants interact
    • Act as an advocate for participants when dealing with other agencies, and support participants to develop their voice and advocate for themselves within systems
    • Maintain clear, professional, timely participant contact notes, service plans, progress updates, and other documentation according to all confidentiality guidelines
    • Attend weekly supervision meetings with the Resource Navigation Manager to review workload, highlight successes, and find solutions to challenges
    • Provide and maintain records for data and tracking reports. Document work, including incident debriefs, client interactions, etc. consistently and accurately
    • Attend mandatory meetings to review and refine protocol, plan hot-spot remediation and upcoming events, as well as all other required training and professional development opportunities
  • Other
    • Participate in meetings, as assigned by supervisor
    • Maintain confidentiality as per agency guidelines
