Job Summary
The Emergency Department Case Management (EDCM) program is an outpatient intensive case management program designed to meet the psychosocial needs of patients who frequently rely on the Medical Emergency Department and or on Inpatient medical services at Zuckerberg General Hospital. Under the general supervision of a clinical supervisor, the Clinical Social Worker I /II is part of an interdisciplinary team who address medical, substance abuse, social service or psychological problems by providing comprehensive assessment, case management / brokerage, individual therapy and crisis intervention services. The CSW I/II provides client care both onsite at EDCM as well as in the community via outreach, home visits, and appointment accompaniment. Please note, EDCM is not housed in the ED but rather on campus of ZSFG.

Please note: The compensation ranges listed online for roles not covered by a bargaining unit agreement are very wide, however a job offer will typically fall in the range of 80% - 120% of the established mid-point. An offer will take into consideration the experience of the final candidate AND the current salary level of individuals working at UCSF in a similar role.
For roles covered by a bargaining unit agreement, there will be specific rules about where a new hire would be placed on the range.

Required Qualifications
* CSW I: Master's degree in social work, psychology or related field from an accredited university and knowledge and abilities essential to the successful performance of the duties assigned to the position.
* CSWII: Master's degree in social work, psychology or related field from an accredited university and possession of the requirements necessary for application for licensure as determined by the California Board of Behavioral Science; and knowledges and abilities essential to the successful performance of the duties assigned to the position.
* Excellent clinical skills.
* Ability to set priorities, work independently, and have excellent follow-up skills.
* Work well in high stress, high volume work setting and efficiently meet deadlines.
* Ability to follow written and oral instructions accurately and be detailed oriented.
* Able to climb stairs and ability to lift up to 50 lbs.
* Excellent documentation skills.
* Experienced working in behavioral health settings or medical settings with ability to further learn concepts and terminology pertinent to mental health and medical services.
* Ability to work sensitively with patients and colleagues from diverse cultural and social class backgrounds.
* Demonstrated punctual attendance and reliability.
* Must be able to successfully pass a background clearance.
* Ability to meet minimum caseload expectations and demonstrate abilities in all basic job duties and required job duties in order to successfully pass six-month probationary period.
Preferred Qualifications
* Experience in case management and prior work in health care setting.
* Experience with adult severely mentally ill and / or people with chronic substance abuse problems who are homeless.
* Experience working with people severe and complex medical conditions.
* Experience working in public health or public agency setting.
* Language capacity for mental health and other services - Spanish.
