GENERAL SUMMARY The LVN will provide direct nursing services for Adult Behavioral Health (ABH), Primary Care, and Intellectual Developmental Disabilities (IDD) consumers, assist with laboratory testing, EKGs, and other medical treatments, maintain pharmaceutical oversight of medication samples, initiate prior authorizations and override forms. The LVN will also complete associated documentation for the consumer and program. ESSENTIAL DUTIES & RESPONSIBILITIES Essential functions are the basic job duties that an employee must be able to perform, with or without reasonable accommodation. Administers medications and provides medication education to patients. Assists with laboratory collection and other medical treatments/interventions. Communicates with collaborating physicians or specialists regarding patient care. Communicates with patients regarding treatment plans. Completes prior authorizations. Conforms to the Texas Nurse I Practice Act and the Board's rules and regulations as well as all Federal, State, and local laws, rules, or regulations affecting the LVN's current area of nursing. Coordinates medication distribution with the Center's pharmacy contract, maintains accurate account of stock medications, controlled substances and consumer medications. Coordinates on medication management and reconciliation with consumers. Coordinates services with pharmacy representatives and/or companies. Educates patients about health maintenance and disease prevention. Ensures the operation of a safe, therapeutic environment. Facilitates referrals to other healthcare professionals and medical facilities. Maintains pharmaceutical oversight of medication samples for specified units participating in the program through assigned unit level staff. Obtains medical records; order labs and obtain and laboratory results. Monitors various supply inventory and ensuring proper rotation of expiring vaccines. Participates as a member of the inter-disciplinary team. Provides administrative support, including answering phones, charting and filing, and ensuring adequate supplies. Provides daily nursing services, including quality nursing care, medication support, educational programs, and case management services for clients. Trains and educates clients on their medical needs. Performs other related duties as required. Specialized Programs/Units IDD: o Administers injectable medications in various homes, facilities or clinics. o Administers nursing care (treatments, monitoring and assisting) to clients in group homes, own home/family home setting and day habilitation centers. o Monitors and reports changes in patient symptoms or behavior. o Participates in quarterly reviews of consumers to assess their current health needs. o Participates in schedule of consults from group homes and host homes. o Provides advice and emotional support to patients and their family members. Child Behavior Health: o Administer long-acting injections to adolescents and monitor for adverse side effects. Primary Care: o Administers various vaccines, oxygen and nebulizer treatments. o Assists with abscess (I&D) drainage ensuring all supplies are present. o Assists with various exams and procedures such as pelvic exams or. Ensures treatment area is supplied with proper instruments. o Conducts spirometry testing. o Conducts vision screenings with Snellen. o Draws and administers insulin and Subcutaneous injections. o Measures lung capacity using peak flow meters. o Participates in procedures such as point of care testing (flu, covid, strep, mono blood glucose and urinalysis. o Performs wound care tasks such as cleaning, dressing and timely changing of wounds. MINIMUM ENTRANCE QUALIFICATIONS Education and Experience High School Diploma or GED equivalency is required. IDD: One (1) year of experience or related experience serving as an LVN in IDD population Licenses or Certifications Current Texas LVN licensure Other Requirements IDD Program: Complies with HCS and TxHmL, ICF and DAHS health related principals. Fact/Act Units: Transport consumers to sites of medical interest (as needed) - i.e. third-party labs, pharmacy, intensive outpatient, etc. Fact/Act Units: If designated a field-based nurse, employee will provide visits to consumers in their homes and the community; clinic-based nurses may be required to assist with this task as needed. CBH: Must be able to pass children's specific credentialing and background check. Must maintain a valid driver's license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company's auto insurance carrier if you drive your vehicle during company business. Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies. Must be able to meet the physical requirements to complete Nonviolent Crisis Intervention (NCI) and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance. Nonviolent Crisis Intervention (NCI) is a proven safe and harm-free method of behavior modification. All consumer-facing employees of CHCS must take NCI courses facilitated by the CHCS Training team within the first 45 days of employment. NCI is intended to support human service professionals in giving aggressive, disruptive, or out-of-control people the best care and welfare possible, even in the most violent situation. NCI training provides staff with the skills to safely recognize and respond to everyday crisis situations that may involve more challenging behaviors. It focuses on prevention and offers proven strategies for safely defusing anxious, hostile or violent behavior at the earliest possible stage. Depending on assigned unit/program, applicants for this position must pass a Criminal Justice Information Systems (CJIS) fingerprint-based background check and maintain CJIS eligibility. Due to CJIS requirements related to system access, the following may result in being disqualified for this position: Felony Convictions, Felony Deferred Adjudication, Class A & B Misdemeanor Deferred Adjudication, Class B Misdemeanor Convictions, an Open Arrest for Any Criminal Offense (Felony or Misdemeanor), and Family Violence Convictions. PREFERRED QUALIFICATIONS Associate's degree in Nursing from an accredited university. Bilingual (English/Spanish) preferred. Language Proficiency Pay (LPP) payments are subject to successful testing, certification by CHCS Payroll, and availability of funding. Funding may be renewed in subsequent fiscal years but is not guaranteed. Previous experience in nursing or other medical fields. One (1) to two (2) years of nursing experience in an acute, fast-paced environment which includes one (1) year in a psychiatric or behavioral health setting. Performing phlebotomy procedures using proficient techniques that avoid repetitive vein puncture. Ability to: Accurately organize and maintain paper documents and electronic files. Accurately set up exam rooms for various medical procedures to include but not limited to: Pap smear, Ear lavage, Pelvic examination and I&D (irrigation and drainage). Address concerns from consumers about medication and treatments. Build and maintain rapport with patients. Conduct assessment of patient condition. Effectively communicate, both verbally and in writing, to clients and staff members. Establish and maintain effective working relationships. Facilitate care coordination (specialty specific IDD, Primary Care). Maintain accurate and complete records. Maintain the confidentiality of information and professional boundaries. Manage the nursing phone line. Meet schedules and deadlines of the work. Thrive in a fast-paced environment. Understand and carry out oral and written directions.
