Position Summary
The Case Manager provides education and employment preparation skills (related to the employee's scope of practice) to criminal justice-involved patients in a community-based facility.

Physical Demands and Work Environment
The work environment is characteristic of a medical clinic environment. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands to handle, or feel. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet.
The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the client's use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bodily fluids (blood, urine) and hazardous chemicals.

Position Summary
The Case Manager provides education and employment preparation skills (related to the employee's scope of practice) to criminal justice-involved patients in a community-based facility.
Required Qualifications
- High School diploma required; Associate or Bachelors degree in related field preferred.
- Two plus years' experience working with offenders OR an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job.
- Knowledge and training in educational and employment opportunities for offenders and the previously incarcerated.
- Knowledge of the local community referrals.
- Individuals who have "lived experience" in recovery are requested to have sustained a minimum of (2) two years of continuous sobriety to maintain the agency's ethical standard for client care.
- Communicate clearly and concisely, both verbally and in writing and demonstrate a high level of listening skills.
- Excellent interpersonal skills including active listening, problem solving, and the ability to remain calm in emotional or stressful situations.
- Computer (PC) literate in general office functions such as word processing, email and database software. Knowledge of Microsoft Office (Word, Excel, Outlook).
- Fluency in additional languages preferred.
- Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.
- Assist and support the Center/Department/Program to meet standards of High Reliability.
- Must have valid driver's license and insurable driving record.
- Provide education services at THRIVE that include: case management, substance abuse education, job readiness, employment services, job coaching, life skills (including financial barriers and training), mentorship, recreational activities, and transition plans.
- Participate in a multidisciplinary team approach. Evidence-based methodology and outcome measures that will be used to evaluate the program and a discussion of how such measurements will provide valid measures of the impact of the program.
- Meet with Department and County staff as required to ensure services continue to be provided in the most effective and efficient manner possible.
- Provide consultation and technical assistance regarding changes or enhancements to program delivery to ensure efficacy.
- Other duties as assigned.
