GENERAL SUMMARY:

The CCHIP Women’s, Pediatric, OB, and Young Adult Navigator will provide peer support to Women’s, Pediatric, OB, and Young Adult Clinic population. The navigator efforts will include communication and documentation of interactions with individuals living with HIV who are currently engaged in care. The Navigator will actively participate in the multidisciplinary clinic team and maintain professional and collaborative communication. The Navigator will promote and facilitate population-specific programming and support groups, provide health education, and assist in the effective transition from adolescents to adult healthcare.

PRINCIPAL DUTIES AND RESPONSIBILITES:

  • Assist clients with engagement in program resources, facilitate program surveys, and facilitate support group programming.
  • Identify and address barriers and/or challenges to care and services that are population-focused.
  • Remind patients of upcoming appointments, provide clinic navigation assistance during appointments, including the first adult clinic appointment for young adults transitioning to adult healthcare, and encourage participation of clients in clinic events.
  • Provide education and information on the transition from adolescent/young adult to adult healthcare.
  • Communicate with the multidisciplinary care team about individual patient needs and concerns. The Navigator should attend clinic, pre-clinic meetings, and client-centered care conferences.
  • Complete training curriculum that includes skill building, professional development, and patient-focused care.
  • Utilize social media to promote programs, raise community awareness, support and outreach to targeted population.
  • Share knowledge and education to improve patient’s knowledge and attitudes toward treatment adherence and social supportive services/activities.
  • Provide Health Literacy to all clients which will help navigate clients through the HIV and primary care system.
  • Data entry and documentation as assigned by case management services coordinator.
  • Assist in creative development of program ideas, including the development of interactive technology and program materials.
  • Participate in all staff development meetings and/or other meetings and trainings as required.
  • Community outreach at mutually agreed upon locations, between client and peer, to ensure follow-up.

QUALIFICATIONS – KNOWLEDGE/SKILLS/EXPERIENCE:

Education:

  • Minimum of a high school diploma or equivalent, and at least 2 years of experience working as a peer educator or lived experience, will be considered.

Experience:

  • Knowledge of HIV treatment and prevention; HIV testing certification is a plus.
