Hospice Social Worker -

A hospice organization in New York is currently seeking a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) to join their hospice program in Queens. In this role, the Hospice Social Worker will will be responsible for providing psychosocial support counseling and other social work related services to palliative care patients and their families in both community homecare and facility settings.

• Schedule: Monday to Friday

• Caseload - 25 cases per week

Responsibilities:
The Hospice Social Worker (LCSW / LMSW) will:

• Serve as a member of the Palliative Care interdisciplinary team
• Provide comprehensive psychosocial report to the team
• Develop the palliative social work plans of care and the goals associated with those plans
• Make recommendations based on patient/family needs for an active caseload
• Refer patients to hospice whenever appropriate
• Perform other duties, as needed

• NYS Licensed Clinical Social Worker (LCSW) and/or Licensed Master Social Worker (LMSW)
• Community Health, Hospice, Hospital or nursing home experience

• Bilingual Spanish, Russian, Cantonese or Mandarin is a plus but not required
