Hospice Homecare Social Worker

We are currently seeking a compassionate and dedicated full-time Social Worker (LMSW or LLMSW) to join our Hospice Homecare team.

Key Responsibilities:

• Conduct initial and ongoing psychosocial assessments of patients and their families
• Provide individual and family counseling to address mental and emotional needs
• Identify special needs related to religious or cultural diversity and provide necessary support
• Evaluate financial or environmental concerns and connect patients and families with community resources
• Participate in IDG meetings and assist with discharge planning when needed

Requirements:

• Master's degree in social work from an accredited program
• Licensed to practice as a Social Worker in the state of employment (LMSW or LLMSW)
• Minimum of one year of healthcare experience, hospice or homecare experience preferred
• Reliable transportation and valid driver's license
