We are searching for a Senior Community Health Nurse to join our team!

Responsibilities:

  • Perform an initial assessment of the client by providing a nursing diagnosis.
  • Design and implement a care plan evaluating the outcomes.
  • Provide for the emotional, physical, and spiritual needs of clients.
  • Administer medications, including parenteral therapy, report pertinent clinical assessment findings to the physician, and collaborate with physicians and other disciplines in case management.
  • Record client data utilizing computerized documentation systems in a timely manner per Agency policy.
  • Collaborate with Intake Staff for client needs in the home.
  • Conduct ongoing training of home health aides as needs arise.
  • Assist with ongoing Agency health status monitoring and documentation for federal and/or state regulatory requirements.
  • Participate in quality assurance/utilization review activities on a monthly basis.
  • Develop and grow the nursing program in designated geographic areas.
  • Facilitate all LNA and PCSP evaluations.
  • Provide supervision of LPN in client care activities when necessary.

Qualifications:

  • Current RN (registered nurse) license with the New Hampshire Board of Nursing required.
  • Five (5) years of experience working in home care.
  • Three (3) years of supervisory experience.
  • Valid driver’s license and reliable transportation.

Location: Candidate must be able to travel between Manchester, Nashua, Stratham, and Rochester, NH locations.
