- Schedule: Mon-Fri, 8:00 AM – 5:00 PM | Full-time, 40 hours/week
- Salary: $210k–$240k/year.
Job Overview
Reporting to the Chief Medical Officer, the Site Physician will lead the provision of high-quality medical care in our Diagnostic and Treatment Center (DTC). You will support an interdisciplinary care team, contribute to strategic improvements in care quality, and serve as a leader in patient-centered, community-focused care.
Key Responsibilities
Clinical Oversight & Collaboration
- Provide and oversee comprehensive patient care, collaborating with NPs and community-based primary care providers.
- Direct the medical management of patients while guiding other interdisciplinary team members.
- Manage a personal caseload as a primary care provider (PCP) within the DTC.
Program Development & Multidisciplinary Support
- Lead "start of care" events and participate in multidisciplinary activities such as case conferences, hospitalization reports, and family meetings.
- Identify and act on opportunities to improve patient care quality, cost efficiency, and utilization.
- Develop and implement ambulatory medical services and utilization review practices.
Educational & Liaison Activities
- Build relationships with subspecialists and community physicians for enhanced care planning.
- Facilitate training for clinical staff and lead medical education initiatives for staff and trainees.
- Develop and participate in preventive health screening, immunization programs, and professional outreach activities.
Direct Patient Care
- Conduct patient assessments and examinations in varied settings, including the DTC and home visits.
- Serve as a clinical resource for staff, providing development and teaching opportunities.
Qualifications
Education
- MD or DO from an accredited medical school, with Board Certification in Internal Medicine or Family Practice; Geriatrics experience or certification preferred.
Licensure
- Current and unrestricted NYS medical license.
Experience
- Residency in Internal Medicine or Family Practice; fellowship or experience in Geriatrics is highly preferred.
- No history of malpractice or licensing issues.
Physical & Cognitive Requirements
This role involves a combination of stationary and active tasks, requiring physical agility and endurance. Key physical requirements include:
- Standing for up to 6 hours/day and sitting for 6-8 hours/day.
- Lifting, pushing, and pulling up to 50 lbs. and safely assisting patients.
- Bending, squatting, walking, and maneuvering stairs as necessary.
- Fine motor skills for operating medical instruments and typing.
- Sharp visual and auditory abilities for patient assessment, documentation, and communication.
- Cognitive strength in processing information, decision-making, and clinical reasoning.
