Join our team as a day shift, full-time, Acute Care Registered Nurse (RN) Case
Manager in Tyler, TX.

* UT Health Tyler, the flagship hospital of the system, has 502 licensed beds specializing in advanced acute care medicine. Our hospital includes a 24-hour ER with Level 1 trauma center, a comprehensive stroke center, more than 500 physicians and a full complement of subspecialty groups.

**Responsibilities**
* Coordinate and advocate for our patients care & create an appropriate discharge plan.
* Review patients charts, and meet with families and healthcare providers to establish the most suitable course of action.
* Should remain in contact with patients and their families during the hospitalization stay.
**Qualifications**
Job Requirements:
* Current RN license in the state of Texas
* Registered Nurse with an Associates, Bachelor’s or Master’s Degree
* Minimum of three (3) years of clinical experience
Preferred Job Requirements:
* Case Management certification
* Experience in utilization review, discharge planning, quality programs
