POSITION SUMMARY
The Central Intake Director develops, coordinates, and directs the facilities pre-admission process in an efficient, timely manner to ensure a positive admission experience. This position is responsible for oversight in all aspects of the pre- admission process including managing members of the central intake team, establishing and maintaining strong, positive relationships with referral sources, responding to admission inquires, and managing insurance verification and authorization processes.
DUTIES AND RESPONSIBILITIES
- Manages the central intake process and team by providing training and coaching in systems and best practices.
- Responsible for gathering all necessary information and completing pre-admission reviews.
- Completes payer verifications to ensure there is payer source prior to admission. Obtain authorization if required by Managed Care, Medicare, and Managed Care Medicaid organizations for all necessary items such as, medication carve-out, DME equipment.
- Monitors referral portals consistently and promptly communicates with case mangers regarding referrals.
- Maintains open and effective communication with all relevant parties, including Director of Care Transition, facility personal, and external healthcare organizations.
- Facilitates regular meetings and updates to discuss goals, challenges, and opportunities for improvement.
- Monitors and improves key metrics such as readmissions, insurance classification, and overall care coordination. Ensure compliance with regulatory and accrediting standards, striving to exceed national best practice benchmarks.
- Promotes the facilities and its services to outside sources such as physicians, discharge planners, business members of the community, churches and schools providing adult education.
- Maintains a thorough knowledge of facility's admission requirements, services and programs, and admissions policies and procedures. Conducts pre-admission interviews with prospective residents/guarantors and explains all services of the nursing facility.

KNOWLEDGE, SKILLS, ABILITIES & QUALIFICATIONS
- Bachelor's or Master's Degree in Nursing from an accredited college/university, or equivalent (preferred).
- 2-4 Years of healthcare leadership in long term care or comparable combinations of education/work related experience
- Proven track record in supervising large teams and managing complex stakeholder relationships in a healthcare setting.
- Project management experience.
- Strong understanding of nursing and medical practices and procedures; laws, regulations and guidelines that pertain to nursing care facilities.
- Ability to adjust priorities, meet deliverables and deadlines and multitask.
- Must have excellent organizational skills and attention to detail.
- Result oriented and data driven.
- Strong analytical, project management, and program development skills.
- Able to adhere to and meet project deadlines, as expected.
