The Wellness Coach 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.The Wellness Coach 2 explains, educates, reinforces, and coaches members on their tailored self-help plans. Identifies problems, sets goals, and determines the individual's health status based on assessment. Establishes goals for each individual regarding educational and health care needs. Communicates with primary care and/or treating physician to request specific health information and/or to provide an update of an individual's health care status. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed.
