CHOR Youth & Family Services is seeking a Case Manager in the Lehigh Valley for our Paths Program located in Breinigsville, PA. Paths is a community-based specialized group treatment home for boys between the ages of 10 and 15. Youth in the program attend community schools through the Parkland School District and participate in a high level of recreational activities as well as being immersed in a highly interactive and positive social milieu.
Summary of Position: Maintains all resident medical and individual service plan portions of records and ensures compliance with standards set forth by all licensing and regulatory bodies (DPW, Managed Care Organizations, & COA). Coordinates all health care appointments and ISP meetings to ensure compliance and standards set forth by all licensing bodies. Facilitates in-home visits and tracks all visitations.

Duties and Responsibilities:
  • Track and maintain the medical portion of client records, hard and digital copies; ensuring compliance with regulations including the procurement of relevant documentation.
  • Review and approve all medical incident reports.
  • Schedule and track all routine, specialist, and follow-up appointments, ensuring compliance with medical regulations set forth by all licensing and regulatory bodies.
  • Secure private and/or medical assistance health insurance information, if not supplied at time of intake.
  • Order, maintain, and monitor prescription medication, ensuring required doses are available for clients as prescribed.
  • Oversee administration of medication, training necessary staff in mandated protocols.
  • Notify parents and/or legal guardians of all client medical appointments and changes in client medications.
  • Individual Service Plan Responsibilities:
    • Keeps and maintains the ISP portion of the resident's record ensuring compliance with regulations set forth by all licensing bodies, referring counties, and Managed Care organizations.
    • Mentors direct care staff with regard to creating and adjusting behavioral goals.
    • Updates daily sheets, ensuring accurate behavioral goals are included
  • Attend weekly supervisor meetings.
  • Attend in-house staff trainings and provides trainings as needed
  • Assist in medical appointment transportation, when necessary.
  • Periodically fill in for Youth Counselors and Supervisors, when necessary.
  • Perform other job duties as assigned.

Education/Experience
Bachelor's degree with a background in the creation and implementation of behavioral modification goals.
