This position will assist the Community First Services & Supports program. Community First Services and Supports (CFSS) is a Minnesota health care program that offers flexible options to meet the unique needs of people. This position will also complete annual reassessments for individuals on CADI, CAC, BI, AC, and EW waivers using an inclusive, person-centered approach that includes race equity.
