This position will assist the Community First Services & Supports program. Community First Services and Supports (CFSS) is a Minnesota health care program that offers flexible options to meet the unique needs of people. This position will also complete annual reassessments for individuals on CADI, CAC, BI, AC, and EW waivers using an inclusive, person-centered approach that includes race equity. One or more years of experience in Home and Community-Based Services (HCBS) is preferred for MnCHOICES Certification.DESIRABLE EXPERIENCE:Experience interacting with and providing customer service to individuals from a variety of cultural and ethnic backgrounds.Certified MnCHOICES Assessor with direct assessment experience.One or more years of experience in Home and Community Based Services (HCBS- waivers) is required for MnCHOICES certification; Direct experience with HCBS and Minnesota Health Care Programs.Demonstrated experience in understanding, implementation, and analysis of person-centered practice and diversity, equity, and inclusion work.Experience and comfort facilitating and/or participating in group work.Ability to efficiently use time and technology to complete all assessment components within required timelines, while applying person centered best practices and ensuring the person understands all resources and options available to them.SELECTION TECHNIQUE: Your training and experience will be rated based on the required application material submitted and the top scoring candidates will be forwarded to the hiring department for further consideration.HOURS / DAYS: This position is Monday – Friday 8:00am – 4:30pm with flexibility and the option to work in-office, hybrid or remotely as approved by the supervisor. A Master's Degree in Social Work or related field may substitute for up to one year of required Social Worker experience.
