Compensation:

Salary: $22.88/hour

Job type: 35 hour work week

The SNAP Fatherhood Initiative Program (SNAP FIP) is an evidence-based parenting education program for previously incarcerated fathers to simultaneously provide services to their children ages 6 to 12. The SNAP Case Manager works directly with families throughout the life cycle of service to provide support and strategies to keep them engaged and on course with their SNAP group, as well as assists with referrals and system navigation for participating fathers. This position is responsible for building solid relationships to work with families to identify, problem-solve, and navigate the unique challenges their family face by providing effective tools and resources. In addition, they also regularly interact with families both in the group as a facilitator, and outside of groups responsible for maintaining all documentation/program files including data entry to pre and post assessment and follow- up calls after services to ensure appropriate resources are provided and program files.

PRIMARY DUTIES/RESPONSIBILITIES:

  • Responsible for the day to day case management of SNAP Fatherhood Initiative ages 6-12 in Manatee and Pinellas Counties.
  • Facilitates SNAP Fatherhood Initiative Program in 13 week cycles
  • Oversees all client files are complete, accurate and comprehensive with updated information recorded in a timely manner.
  • Completes intake assessments for SNAP Fathers Initiative Program including all required assessments associated in the intake process.
  • Adapts the values of Family Resources and the SNAP program and works to implement the SNAP curriculum in accordance to SNAP programming.
  • Attend the 5-day core SNAP training (free, provided by Florida Network).
  • Enter case information into databases including Netmis, and ASEBA.
  • Monitors youth and family's progress through weekly compliance calls.
  • Conducts community visits with children at school to enhance SNAP success.
  • Participates in pre briefing and debriefing meetings weekly with other SNAP Team members.
  • Liaises with relevant community agencies to include schools, community based agencies, and Police Departments to enhance outreach.
  • Participate in additional SNAP meetings as necessary and
  • Participates in monthly compliance calls with the Florida Network, if needed.
  • Completes staff training as required by SNAP.

  • Prepares technology that is required for weekly group delivery.
  • Completes fidelity adherence checklist for weekly groups in a timely manner.
  • Engages in quarterly review process and other ongoing quality improvement measures to assess program performance and to ensure all contractual goals are met and required outcomes are obtained.
  • Assists supervisor in producing overall monthly, quarterly, and semi-annual reports.
  • Willingness to work nontraditional business hours including evenings up to 8pm. Evening hours are mandatory for this position and is dependent on the schedule of the SNAP groups.
  • Must have reliable transportation.
  • Willing to travel to both Manatee and Pinellas locations as needed.
  • Works under the supervision of the SNAP Supervisor and participates in staff meetings and supervisions as required.
  • Must be willing to be recorded during group facilitation.
  • Other duties as assigned.

QUALIFICATIONS REQUIRED:

  • Bachelor's degree from an accredited university preferred, with emphasis in areas of psychology, social work or related field.
  • Minimum of three years' experience working with youth and their families.
  • Ability to develop and maintain effective working relationships with individuals from a variety of cultural and ethnic backgrounds. Must demonstrate awareness and acceptance of cultural differences of clients, staff and community contacts.
  • Possess excellent interpersonal, communication, organizational and time management skills.
  • Valid Florida's driver's license, clear driving record and current automobile insurance.
  • Background compliant with requirements of the Department of Juvenile Justice.
  • Must possess a cellular phone to be used for agency business.
  • The ability to lift 15 pounds regularly.
  • The ability to respond quickly to sounds.
  • The ability to see and respond to dangerous situations.
  • Frequent standing and/or sitting.
  • Other physical demands that are required are as follows: Stooping and/or kneeling, reaching, talking, hearing, seeing.
  • Schedule: Non-Traditional business hours up to 8pm in Pinellas and Manatee County. Must also be willing to work weekends if needed.
