The Care Coordinator provides support to primary care providers (PCPs) and families in our community helping to manage and coordinate medical and mental health care and other services and supports along with the Pediatric Access Line (PAL). Having extensive knowledge of local resources and/or lived experience, the Care Coordinator successfully links PCPs and families to the appropriate behavioral health services and supports based on their specific needs and insurance status. This position helps clients by identifying service needs and assisting with the utilization of service providers and community resources that strengthen and enhance the quality of their lives.
PLEASE NOTE: This role is a grant funded role. These positions are subject to continuation based on availability of grant funds, and the employee will be terminated without right of appeal when such funds are no longer available. Funding for this position is approved through September, 2025, and continuation of this position is subject to ongoing grant funding.

- Develop and maintain relationships with intake coordinators and clinic managers at community mental health agencies, clinics, and organizations.
- Research and maintain accurate and up to date lists of local mental health resources and their availability, including mental health providers, family support services, community resources, peer support programs in the PAL database. Each resource should be verified at least annually; availability of resources may vary by region.
- Answer PAL PCP calls/emails during normal business hours (9-5 M-F, except holidays) and direct to appropriate resources. Respond to PCP requests with appropriate regional mental health resources within 24 hours, including practice information such as participation with insurance and wait time for appointments.
- When necessary, work directly with families to help facilitate and coordinate referrals to needed services.
- Provide follow-up calls to referred families within designated time frames in order to ensure continuity of care.
- Provide feedback as requested to PCPs on status of family engagement and services.
- Record and log information from daily calls into PAL electronic medical record, DrChrono.
- Participate regularly in community-based meetings to stay current on new and evolving resources to support youth and families served by the PAL.
- Adhere to HIPAA privacy policies and hospital/health center regulations.
- Will be supervised by the PAL Medical Director.
- Functions effectively in a Matrix Management environment.
- Completes any duties and special assignments as requested.
- Minimum of two (2) years of experience in Social Services, Education System, Child/Adolescent Mental Health Programs/Services, and/or related field required
- High School Diploma or GED equivalency required
- Bachelors Degree in Social Work, Human Services, and/or related field preferred
- Bilingual is a plus, preferably Spanish
- Knowledge and experience navigating healthcare systems and/or mental health services strongly preferred
- Knowledge and skills in coordinating referrals for various insurance types and providers
- Knowledge and experience using electronic health records software (i.e. Epic Systems or Dr. Chrono)
- Strong customer service skills, particularly while engaging over the phone
- Strong organizational abilities including effective time management skills, able to manage multiple tasks at the same time and respond to requests in a timely matter
- Strong interpersonal skills in order to effectively engage with a diverse range of providers, agencies, and families
- Demonstrated proficiency in Microsoft Office (Word, Excel, etc.) and Google Suite (Gmail, Docs)
- Demonstrates flexibility and willingness to learn
- Strong critical thinking and problem solving skills
- Excellent verbal and written communication skills to thoroughly convey and document work on an ongoing basis
- Must be able to multitask and prioritize work in a fast paced environment
- Must be able to maintain confidentiality
- Must be able to pay close attention to details
- May include standing, sitting, and/or walking for extended periods
