Overview

The Ambulatory Nurse Care Manager, Primary Care is responsible for coordinating care for an identified population of at-risk and emerging risk patients followed in the primary care clinics. The Ambulatory Care Manager will follow the philosophies of case management to provide care coordination services in order to make sure that patients receive well-coordinated care along the health care continuum and promote quality care through appropriate, cost effective interventions. The Ambulatory Care Manager will promote preventative care and address condition specific needs of the patient population. This position requires problem-solving, communication with patients, providers and ancillary staff and clinical expertise to address the patient's barriers to care.

Responsibilities

  • Provides care management and coordination of services for patients utilizing the case management process.
  • Maintains timely documentation in the EHR to enhance communication and collaboration across the healthcare system.
  • Supports the standards of the Patient Centered Medical Home (PCMH).
  • Meets the educational needs of the patient and family and advocates for their needs.

Qualifications

  • Bachelor's Degree BSN and 3-5 years experience in primary care.
  • One of the following: Licensed RN - MO, Registered Nurse Multistate License Missouri required upon hire.
  • Licensed RN - Kansas, Registered Nurse Multistate License Kansas required within 90 days.
  • One of the following: Certified Case Manager, American Case Management required within 2 years.
  • Refer to Nursing and Advanced Practice Provide Licensure and Certification policy for required Life Support Certifications Required Upon Hire.
  • Specialty Certification relevant to unit's patient population (determined by Nurse Manager/Director).

Starting Pay

Our pay ranges are market competitive. The pay range for this job begins at $36.75/hr, but your salaried offer will be determined based upon your education and experience.

Remote Work/Work from Home

This position is not eligible to work remotely, which means that the person hired will be required to work onsite at one of our Children's Mercy locations and may not work from home.
