This is a full-time position, M-F with rotating Saturdays

Position Summary

The General Responsibilities:

The Case Management staff meet each client where they are at, assessing both strengths and needs. Together they develop a goal plan to reach the client's greatest level of self-sufficiency. Case Managers help clients access health and mental health services; entitlement programs such as Food Stamps, TANF, SSI/SSD; other needed Community Resources, Coordinate with family members and other supports in client's life to develop wrap around services to build a strong foundation; Link clients to employment services and housing opportunities; Support clients for up to six months.

Essential Functions of the Position:

1. To support and promote the mission statement and values of Street 2 Feet and RHD, Inc.

2. To engage people in a trusting and supportive relationship, providing emotional support and focusing on target goals.

3. To collaborate with individuals and others in the completion and review of a strengths based assessment

4. To assist the person to develop an individualized, self-sufficiency service plan based on the specific needs of the individual.

5. Maintain a current referral list for clients.

6. Maintain relationships with the various programs in our community to promote our service.

7. Engaging clients in a trusting and supportive treatment partnership, providing emotional support while maintaining professional boundaries.

8. Maintain required records accurately, comprehensively and in a timely manner, including data entry into the Homeless Management Information System.

9. Keeps required statistics and submits reports as requested.

10. Cooperates with, participates in and supports the adherence to all internal policies, procedures and practices, and compliance with all regulatory requirements.

11. Provides liaison with family members, community agencies, advocacy persons/groups and hospitals when appropriate.

12. To work in a collaborative and collegial manner with other staff both intra- and inter-agency in assisting both the program's clients and the program to meet identified goals.

13. Insures the confidentiality of all client and/or employee-related information.

14. To maintain one's own physical, mental and emotional well-being to maximize the healthy functioning of the program.

15. Attend Monroe County Homeless Advisory Board Meetings and community meetings/ events.

16. Participate in the development and implementation of program fundraising events and activities.

17. Participate in public speaking events to bring program awareness to the community.

18. Attend program fundraisers and community events.

19. Represent RHD and the program out in the community with professional demeanor.

20. Maintain a valid PA Driver License and active car insurance.

21. Report in writing any conflict of interest in working with clients of the Day Center that you may know personally

22. Participate in weekly supervisions with Supervisor to discuss client concerns/needs.

Performs tasks which are supportive in nature to the essential functions of the job, but which may be altered or re-designed depending upon individual circumstances.

Basic Qualifications

Education/Training:

Approximately two years experience in the social services field preferred

Must have a Bachelor degree in Human Service field (preferably in social work); Certification in Drug & Alcohol a plus.

Valid Drivers License and active vehicle insurance

Must be able to pass a background Check and Child Abuse Clearance case manager will determine resources needed to meet the individualized needs of the participant. The case manager will take all necessary actions in assuring the implementation of the service treatment plan including serving as an advocate for the participant to enable him/her to receive needed services; identification of program needs, service gaps and recommending strategies. A commitment to the RHD values should be demonstrated as job duties are performed.

Essential Duties and Functions

Team-Based Planning

  • Act as part of a multidisciplinary team designed to provide services from a holistic and Values based perspective.
  • Assist in the development of recovery plans and reviews in collaboration with the program participant, clinical and medical staff and other stakeholders as determined in collaboration with program participant.
  • Develop and maintain relationships with community agencies in order to assist program participants in connecting with all available resources.

Direct Client Care

  • Provide community-based services to program participants focusing on education, connection with resources, community interaction and achievement of recovery plan goals.

Administrative Functions

  • Complete all assessments and documentation as required by RHD, funder, licensing/ accreditation entities.
  • Other duties as assigned

Essential Duties Addendum

FASST Connections (titled Intensive Case Managers per funder). Staff are required to ensure 24-hour access to Intensive Case Management services.

Lower Merion Counseling & Mobile Services, (titled Administrative Case Manager per funder) provides short-term service and provides assistance to connect families and youth to MA and other services and supports.

  • Assess and respond to needs and service eligibility of individuals in service/potential individuals in service, both in-office (approximately 60% of the time) and in the community (approximately 40% of the time) and with creative flexibility.
  • Provide prompt screening and referral to appropriate treatment and supports.
  • Assist with other resource/benefit acquisition and coordination as appropriate (Medicaid, Social Security, housing, social services, transportation, employment, medication assistance, etc) and develop and maintain relationships with community agencies in order to assist program participants in connecting with all available resources.
  • Provide information and referrals for behavioral health and other human services, including other agencies, RHD programs, and other county agencies/departments. Collaborate with and report to these human services as necessary.

Qualifications

  • Bachelor's Degree in Human Services or related field.
  • Intermediate computer skills
  • One year Case Management experience preferred
  • Two years' general work experience providing support to individuals in behavioral health, Addiction Recovery or IDD services required.
  • Valid driver's license

Working conditions

Additional Special Working Conditions:

  • Working out in community within the service delivery area of the program. Required to drive program participants to appointments.
