Job Summary

We are seeking a skilled and compassionate Home Health Nurse to join our team at the University of New Mexico - Hospitals. As a Home Health Nurse, you will provide direct nursing care to patients and families in their homes, coordinating home healthcare activities and ensuring adherence to hospital and departmental policies and procedures.

Key Responsibilities

  • Assess patient's physical, psychological, social, and environmental status to develop a tailored patient care plan.
  • Provide nursing care and coordinate ancillary services to achieve treatment plan goals, supervising ancillary nursing staff as needed.
  • Administer medications and perform treatments for assigned patients, in accordance with medical and nursing protocols.
  • Report pertinent observations and reactions regarding changes in patient status to the appropriate team member.
  • Provide follow-up to ensure problem resolution, recording observations and patient care activities.
  • Evaluate patient's response to approved plan of treatment, modifying treatment plan as necessary.
  • Interpret agency philosophy and policies to patients, families, and other groups, utilizing agency and community resources to benefit the client.

Requirements

  • 1 year of directly related experience as a Registered Nurse.
  • CPR for Healthcare/BLS Provider or Professional Rescuer certification within 30 days.
  • RN licensure in New Mexico or reciprocal agreement by New Mexico.

Working Conditions

This position involves sedentary work, with occasional walking or standing. The work environment is office-based with minimal hazards and physical risks.
