This job was posted by : For more information, please see: SUMMARY:The Care Coordinator position works under the direct supervision of the Regional Manager with general supervision from the Director of Client Services. Care Coordinators are responsible for locating, coordinating, and monitoring services and resources for all individuals over the age of 60, and individuals age 21 through 64 with a physical disability, to include, minorities, low-income and rural senior citizens, as needed. Provides support, information, assistance, and education to individuals which are providing care to elderly family members.AGENCY EXPECTATION OF EMPLOYEE:Adheres to Agency Policy and ProceduresActs as a role model within and outside the AgencyPerforms duties as workload necessitatesMaintains a positive and respectful attitudeCommunicates regularly with supervisor about department issuesDemonstrates flexible and efficient time management and ability to prioritize workloadConsistently reports to work on time prepared to perform duties of positionMeets department productivity standardsDUTIES AND RESPONSIBILITIES:Respond to referrals to assist all individuals over the age of 60 and/or with disabilities who need care coordination, to include all minorities, low-income and rural senior citizens.Assisting above identified clients with programs of which they meet eligibility, including ARChoices, Targeted Case Management, State Case Management, MIPPA, SHIIP, Caregiver and VAConduct needs assessments and develops service plansCoordinate and monitor services and client situationComplete required documentation and reportingProvide information, referral services and application assistance on a variety of programs and benefits to clients, families, and caregiversAdvocate for the rights and needs of the senior citizen populationAdvocate for clients & assist them with gaining access to medical, social, educational, and other services as appropriateAssist in identifying and documenting service gaps in designated service areaConduct outreach in designated service area to inform all individuals over the age of 60 of available services and resourcesCoordinate with other AAA programs, service providers, and agencies providing services to individuals in the target populationComplete required documentation and reporting in a timely mannerAttend training as provided or requiredConduct Medicare Counselling in partnership with the SHIIP programInstall, monitor/troubleshoot, and maintain/replace Personal Emergency Response Systems (PERS)Assist in the marketing, development and growth of the Private Pay and Durable/Non-durable Goods ProgramsProvide comprehensive and accurate Information and Assistance to clients, family of clients, caregivers, staff, and other agencies.Attend Interagency Meetings in your service area as assignedAssists in ensuring that resource files and resource directory are updatedDuties and responsibilities may be added, deleted, or changed at any time at the discretion of management, formally or informally, either verbally or in writing.
